经会阴四维超声在经产妇盆底康复治疗中的临床意义

摘　　要：目的探讨经会阴四维超声在经产妇盆底康复治疗中的临床意义。方法以本院2018年5月1日至2019年4月30日收治的80例女性盆底功能障碍性疾病(FPFD)患者为研究对象,随机将其分为对照组与试验组,各40例。对照组予以常规产后健康教育+产后保健操,试验组在对照组基础上予以针对性的康复治疗。经会阴四维超声监测患者缩肛、静息以及瓦氏动作时的盆膈裂孔面积、盆膈裂孔周长、盆膈裂孔前后径及耻骨直肠肌宽度,并比较两组PFDI-20评分。结果产后12周,试验组缩肛、静息以及瓦氏动作时盆膈裂孔面积、盆膈裂孔周长以及盆膈裂孔前后径均小于对照组,PFDI-20评分低于对照组(P<0.05)。结论经产妇盆底康复治疗中进行经会阴四维超声检查,可有效评估康复治疗效果,改善盆底功能,具有重要的指导意义。Objective To explore the clinical significance of transperineal four-dimensional ultrasound in pelvic floor rehabilitation treatment of multipara.Methods Eighty patients with female pelvic floor dysfunction(FPFD)admitted in our hospital from May 1,2018 to April 30,2019 were seted as the study objects and randomly divided into control group and experimental group,with 40 cases in each group.The control group were given routine postpartum health education+postpartum health exercise,and the experimental group were given targeted rehabilitation treatment on the basis of the control group.The pelvic diaphragm hole area,pelvic diaphragm hole circumference,the anterior and posterior diameter of the pelvic diaphragm hole and the puborectalis muscle width were monitored by perineal four-dimensional ultrasound during anal shrinkage,resting and Waals moves,and PFDI-20 scores were compared between the two groups.Results At 12 weeks postpartum,the puborectalis muscle width,pelvic diaphragm hole area,pelvic diaphragm hole circumference,the anterior and posterior diameter of the pelvic diaphragm hole in the experimental group at the anal contraction,resting and Waals moves were smaller than those in the control group,and PFDI-20 score was lower than that in the control group(P<0.05).Conclusion Transperineal four-dimensional ultrasonography during pelvic floor rehabilitation treatment of multipara can effectively evaluate the effect of rehabilitation treatment and improve pelvic floor function,which has important guiding significance.
