超声在孕早期鉴别诊断正常宫内妊娠与剖宫产瘢痕妊娠的价值

摘　　要：目的探索超声在孕早期鉴别诊断正常宫内妊娠(IUP)与剖宫产瘢痕妊娠(CSP)的价值。方法选择2017年5月16日至2018年5月16日收治的89例疑似CSP产妇作为研究对象,均采用腹部联合阴道超声检查,分析超声在CSP分型中诊断正确率以及诊断价值。结果超声对孕囊型诊断正确率为95.83%,对宫腔型诊断正确率为96.77%,对包块型诊断正确率为94.44%。超声诊断CSP的灵敏度为94.52%(69/73),特异度为93.75%(15/16),漏诊率为5.48%(4/73),误诊率为6.25%(1/16)。结论超声对鉴别CSP、IUP具有显著作用,可为疾病诊断、治疗方案拟定、预后评估提供科学依据。Objective To explore the value of ultrasound in the differential diagnosis of normal intrauterine pregnancy(IUP)and cesarean scar pregnancy(CSP)in early pregnancy.Methods From May 16,2017 to May 16,2018,89 pregnant women with suspected CSP were selected as the study objects,and all were examined by abdominal combined vaginal ultrasound.The diagnostic accuracy in CSP classification and diagnostic value of ultrasound were analyzed.Results The diagnostic accuracy of ultrasound was 95.83%for gestational sac type,96.77%for uterine cavity type and 94.44%for mass type.The sensitivity,specificity,missed diagnosis and misdiagnosis ultrasonic diagnosis of CSP were 94.52%(69/73),93.75%(15/16),5.48%(4/73)and 6.25%(1/16)respectively.Conclusion Ultrasound plays a significant role in the differentiation of CSP and IUP,which can provide scientific basis for disease diagnosis,treatment plan formulation and prognosis evaluation.
