负压引流技术在淋巴结结核术后中的应用及护理要点

摘　　要：目的探讨负压引流技术(VSD)在淋巴结结核术后的应用及护理要点。方法回顾性分析72例在西安市胸科医院接受淋巴结结核术后VSD持续引流患者的临床资料,针对引流管制定、实施标准化的护理措施,分析护理效果。结果 72例患者伤口均愈合良好,引流时间14~20 d。引流管去除负压引流后16例患者进行二期缝合,56例患者去除引流后经换药后逐渐愈合,伤口愈合时间18~28 d。所有患者经过引流治疗均治愈,未出现切口混合感染、出血等并发症。引流过程中最常见的并发症为引流管堵塞和引流装置漏气。术后48、96 h,引流管堵塞发生率分别为55.6%和69.4%,引流装置漏气发生率分别为16.7%和66.7%,VSD引流平均更换次数为(4.1±1.7)次。结论淋巴结结核术后VSD负压吸引安全、有效,有利于促进疾病的恢复。使用过程中应结合疾病特点,制定对应的护理措施。Objective To investigate the application and nursing points of vacuum sealing drainage(VSD)in the treatment of lymph node tuberculosis after operation.Methods The clinical data of 72 patients who received VSD continuous drainage of lymph node tuberculosis after operation in Xi'an chest hospital were analyzed retrospectively.The standardized nursing measures were formulated and implemented for the drainage tube,and the nursing effect was analyzed.Results The wound healing of 72 patients were good,and the drainage time were 14-20 days.There were 16 patients underwent secondary suture and 56 patients'wound healed gradually by dress changing after the removal of drainage.The wound healing time was 18-28 days.All the patients were cured by drainage without complications such as incision mixed infection and bleeding.The most common complications during drainage were drainage tube blockage and leakage of the drainage device.After 48 hours and 96 hours,the incidence of drainage tube blockage were 55.6%and 69.4%,and the incidence of leakage of the drainage device were 16.7%and 66.7%,respectively.The average of number of replacement of the VSD drainage device was(4.1±1.7).Conclusion VSD negative pressure suction of lymph node tuberculosis after operation is safe and effective,which is conducive to promoting the recovery of the disease.The corresponding nursing measures should be made according to the characteristics of the disease during the use.
