第十二肋下神经阻滞治疗肾绞痛的效果分析

摘　　要：目的探讨第十二肋下神经阻滞治疗肾绞痛的临床效果。方法选择2018年1月至2018年9月江门市中心医院收治的50例肾绞痛患者,按照随机数字表法将其分成观察组和对照组,每组25例。治疗组采用10 ml 2%利多卡因经十二肋下行神经阻滞,对照组采用臀部肌肉注射双氯芬酸钠。采用视觉模拟评分法(VAS)评价患者的疼痛强度。比较两组治疗后120 min内不同时间点VAS评分的差异。如果患者在观察期间出现肾绞痛复发,均按照原组别的治疗方案重新治疗。记录比较两组VAS评分并进行统计学分析。结果观察组不同时间段VAS评分与对照组比较,差异有统计学意义(P<0.01)。共有19例患者在治疗后出现肾绞痛复发,其中观察组7例,对照组12例。两组复发患者治疗后VAS评分比较差异有统计学意义(P<0.01)。结论第十二肋下神经阻滞治疗肾绞痛显效快,疼痛缓解时间长,药物不良反应少,且费用低廉,值得临床推广应用。Objective To explore the clinical effect of the 12th subcostal nerve block in the treatment of renal colic.Methods Fifty patients with renal colic treated in Jiangmen Central Hospital from January 2018 to September 2018 were divided into observation group and control group according to the method of random number table,25 cases in each group.In the treatment group,10 ml of 2%lidocaine was used to block the nerves through the 12th rib,while in the control group,diclofenac sodium was injected into the buttock muscles.The pain intensity of the patients were evaluated by visual simulation scoring(VAS).The VAS scores between the two groups were compared at different time points within 120 minutes after treatment.If the patient relapsed with renal colic during observation,it was retreated in accordance with the treatment plan of the original group.The VAS scores of the two groups were recorded and compared and statistically analyzed.Results Compared with the control group,the VAS scores of the observation group in different time periods were statistically significant(P<0.01).There were 19 patients with recurrent renal colic after treatment,7 in the observation group and 12 in the control group.There was significant difference in VAS score between the two groups(P<0.01).Conclusions The 12th subcostal nerve block is effective in the treatment of renal colic,with long pain relief time,less adverse drug reactions,and low cost,which is worthy of clinical application.
