健脾清热化痰汤治疗痰热壅肺证小儿急性支气管炎临床研究

摘　　要：目的:观察健脾清热化痰汤治疗痰热壅肺证小儿急性支气管炎的临床疗效及对血清炎症因子水平的影响。方法:将痰热壅肺证急性支气管炎患儿156例随机分为2组,每组78例;在常规西药治疗的基础上,对照组采用小儿消积止咳口服液治疗,观察组在对照组治疗的基础上加用健脾清热化痰汤治疗;2组均连续治疗7 d,观察比较2组临床疗效、临床症状和体征消失时间,比较2组治疗前后中医证候积分、血清C-反应蛋白(CRP)及肿瘤坏死因子-α(TNF-α)水平变化。结果:总有效率观察组为97.44%,对照组为88.46%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组患儿主症积分、次症积分以及总积分均较治疗前明显降低(P<0.05);且观察组各项积分均低于对照组(P<0.05)。治疗后,观察组患儿体温复常、喘憋、咳嗽、哮鸣音以及湿啰音消失时间均明显低于对照组,差异有统计学意义(P<0.05)。治疗后,2组患儿血清CRP、TNF-α水平均较治疗前明显降低(P<0.05),且观察组CRP、TNF-α水平均低于对照组(P<0.05)。结论:健脾清热化痰汤可快速改善小儿急性支气管炎痰热壅肺证患儿的临床症状和体征,改善机体炎症反应,临床疗效显著。Objective: To observe the clinical effect of Jianpi Qingre Huatan tang for acute bronchitis in children with syndrome of phlegm-heat obstructing lung and its effect on inflammatory factors in serum. Methods:A total of 156 cases of children with acute bronchitis with syndrome of phlegm-heat obstructing lung were randomly divided into two groups,78 cases in each group. Based on routine western medicine,the control group was treated with Xiao’er Xiaoji Zhike oral liquid,and the observation group was additionally treated with Jianpi Qingre Huatan tang based on the treatment of the control group.Both groups were treated for seven days. The clinical effect and the disappearance time of clinical symptoms and signs in the two groups were observed and compared. Before and after treatment,the changes of Chinese medicine syndrome scores and levels of C-reactive protein(CRP) and tumor necrosis factor-α(TNF-α) in serum in the two groups were compared.Results:The total effective rate was 97.44% in the observation group and 88.46% in the control group, the difference being significant(P<0.05). After treatment, the scores of main and secondary symptoms and the total scores in the two groups were significantly decreased when compared with those before treatment(P<0.05);the above scores in the observation group were lower than those in the control group(P<0.05). After treatment, the time for temperature to return to normal and the disappearance time of wheezes, cough, wheezing sound and moist rale in the observation group were significantly earlier than those in the control group,the difference being significant(P<0.05). After treatment, the levels of CRP and TNF-αin serum in the two groups were significantly decreased when compared with those before treatment(P<0.05), and the above levels in the observation group were lower than those in the control group(P<0.05).
