疏肝活血止痛散外敷联合常规疗法治疗非特异性肋软骨炎临床研究

摘　　要：目的:观察疏肝活血止痛散外敷联合常规疗法治疗非特异性肋软骨炎的疗效。方法:将90例气滞血瘀证患者随机分为对照组和观察组各45例。2组均口服布洛芬缓释胶囊、复合维生素B片,并施予常规护理措施。观察组同期采用疏肝活血止痛散外敷患处,2组均连续治疗4周。治疗前后应用视觉模拟评分法(VAS)评价胸痛程度,应用健康相关生活质量问卷(SF-12)评价生活质量,应用阿森斯失眠量表(AIS)评价睡眠质量。比较2组的临床疗效与护理满意度。结果:治疗后,观察组临床疗效优于对照组(P<0.05)。2组SF-12 8个维度的评分均较治疗前升高(P<0.01),观察组8个维度的评分均高于对照组(P<0.01)。2组AIS评分均较治疗前下降(P<0.01),观察组AIS评分低于对照组(P<0.05)。治疗7、14、21、28 d,2组VAS评分均较前一个时间点下降(P<0.01),观察组4个时间点的VAS评分均低于对照组(P<0.01)。观察组护理满意度高于对照组(P<0.05)。结论:在常规西药治疗与护理的基础上加用疏肝活血止痛散外敷治疗非特异性肋软骨炎,可起到较好的止痛效果,减轻了疼痛对睡眠的影响,改善了患者的生活质量。Objective: To observe the curative effect of external application with Shugan Huoxue Zhitong powder combined with routine therapy for nonspecific costochondritis. Methods:A total of 90 cases of patients with blood stasis due to qi stagnation syndrome were randomly divided into the control group and the observation group,45 cases in each group.Both groups were given oral administration of ibuprofen sustained-release capsules and compound vitamin B tablets,and routine nursing measures;at the same time,the observation group was additionally given external application with Shugan Huoxue Zhitong powder on the affected part. Both groups were treated for four weeks. Before and after treatment, the degree of chest pain was evaluated by Visual Analogue Scale(VAS),quality of life by 12-Item Short Form Health Survey(SF-12),and sleep quality by Athens Insomnia Scale(AIS). The clinical effect and nursing satisfaction in the two groups were compared.Results:After treatment,the clinical effect in the observation group was better than that in the control group(P<0.05). The scores of eight dimensions in SF-12 in the two groups were increased when compared with those before treatment(P<0.01),and the scores in the observation group were higher than those in the control group(P<0.01). The AIS scores in the two groups were decreased when compared with those before treatment(P<0.01),and the score in the observation group was lower than that in the control group(P<0.01). After treatment for 7,14,21 and 28 days,the VAS scores in the two groups were decreased when compared with those in the previous time point respectively(P<0.01),and the scores at four time points in the observation group were lower than those in the control group(P<0.01). The nursing satisfaction in the observation group was higher than that in the control group(P<0.05). Conclusion:Based on routine western medicine and nursing,the additional treatment of external application with Shugan Huoxue Zhitong powder for nonspecific costochondritis has good antalgic effect,and
