摘 要:目的研究高危型人乳头瘤病毒(HR-HPV)感染持续女性阴道微生态状态,为HR-HPV持续感染的防治提供参考。方法选取2016年1月至2019年1月我院收治的286例HR-HPV持续感染女性为持续感染组,选取同期196例健康女性为对照组,采用bPR-2014A阴道微生态检测体系评价两组研究对象阴道微生态情况。结果持续感染组患者阴道菌群密集度以Ⅳ级为主,菌群多样性以Ⅱ-Ⅳ级为主,其中占优势的革兰阳性大杆菌、革兰阳性球菌、革兰阴性短杆菌比例差异不大。对照组女性阴道菌群密集度以Ⅱ-Ⅲ级为主,菌群多样性以Ⅱ-Ⅲ级为主,优势菌以革兰阳性大杆菌为主。酶学监测显示持续感染组患者过氧化氢缺乏率、唾液酸苷酶阳性率、白细胞脂酶阳性率均高于对照组,差异均有统计学意义(t=4.396、23.344、77.241,P=0.036、<0.001、<0.001)。两组对象Nugent评分、pH构成以及β-葡萄糖醛酸酶阳性率、乙酰氨基葡萄糖苷酶阳性率差异均无统计学意义(t=5.003、1.027、2.803、0.857,P=0.082、0.311、0.094、0.355)。结论宫颈癌防治过程中对于HR-HPV持续感染的治疗应注重维系阴道微生态的平衡。Objective To observe the vaginal microecological status of high risk human papillomavirus(HR-HPV)infected women,and provide clinical data for the prevention and treatment of persistent HR-HPV infection.Methods A total of 286 women with persistent HR-HPV infection from January 2016 to January 2019 were selected as the persistent infection group,with 196 healthy women as the control group.The vaginal microecology of the two groups was evaluated by using bpr-2014 a system.Results In the persistent infection group,the concentration of bacteria was mainly at grade IV,and the diversity of bacteria was mainly at gradeⅡ-Ⅳ;the proportions of dominant bacteria,Gram-positive cocci and Gram-negative bacilli were not significantly different.In the control group,the concentration of bacteria was mainly at gradeⅡ-Ⅲ,and the diversity of bacteria was mainly at gradeⅡ-Ⅲ;the dominant bacteria was mainly Gram-positive bacilli.Enzyme monitoring showed that the rates of hydrogen peroxide deficiency,positive sialidosidase and positive leukocyte lipase in the persistent infection group were higher than those in the control group(t=4.396,23.344,77.241;P=0.036,<0.001,<0.001).There were no significant differences in Nugent score,pH,β-glucuronidase positive rate and acetyl glucosidase positive rate between the two groups(t=5.003,1.027,2.803,0.857;P=0.082,0.311,0.094,0.355).Conclusion In the prevention and treatment of cervical cancer,we should pay attention to maintain the balance of vaginal microecological homeostasis during treatment of persistent HR-HPV infection.
