摘　　要：目的探究股内收肌切断术+髋关节置换术对晚期股骨头坏死(ONFH)患者近期预后和关节功能的影响。方法回顾性收集2017年6月至2019年1月收治的晚期ONFH行髋关节置换术的60例患者,其中行单纯髋关节置换术(对照组)34例,行股内收肌切断术+髋关节置换术(观察组)26例。比较两组手术情况,并于术前、术后5 d及术后3个月分别评价视觉模拟评分(VAS)、髋关节外展活动度、Harris评分和关节功能恢复情况。结果与对照组比较,术后5 d及术后3个月观察组VAS评分显著降低,髋关节外展活动度、Harris评分及关节功能优良率显著升高(均P<0.05);两组患者术后伤口均Ⅰ期愈合。结论晚期ONFH患者采用股内收肌切断术+髋关节置换术治疗可有效减轻患者疼痛,改善髋关节外展受限及关节功能,患者术后恢复良好。Objective To investigate the effect of femoral adductor severance and hip joint replacement on short-term prognosis and joint function in patients with osteonecrosis of femoral head(ONFH).Methods A retrospective study was conducted on 60 patients with advanced ONFH who underwent hip joint replacement between June 2017 and January 2019,which included 34 cases undergoing hip joint replacement(control group)and 26 undergoing femoral adductor severance and hip joint replacement(study group).The two groups were compared for the operations,visual analog scale(VAS)at baseline,5 days and 3 months after surgery,hip abduction,Harris score,and joint function recovery.Results Compared with the control group,the study group showed significantly lower VAS at 5 days and 3 months after surgery,greater magnitude of hip abduction,higher Harris score and better joint function(all P<0.05).The wounds of patients in both groups were healed at stageⅠ.Conclusion For advanced ONFH,femoral adductor severance and hip joint replacement can effectively alleviate pain,improve hip abduction limitation and joint function,and result in favorable postoperative recovery.
