摘 要:目的利用有限元分析法探讨股骨假体柄-骨接触率及接触位置对非骨水泥型人工髋关节置换术后初始稳定性的影响。方法基于一名成人志愿者股骨的DICOM格式CT图像数据,综合应用Mimics、SolidWorks软件构建股骨、假体柄实体模型,并定位装配,导入HyperMesh软件划分体网格,返回Mimics软件使用CT灰度值赋值法进行赋材质、建立人工髋关节置换术后有限元模型。最后在ABAQUS软件中设定15种不同柄-骨接触率的术后模型,然后进行攀爬楼梯仿真分析,分别计算柄-骨界面12171个节点的微动。结果15例模型中有8例初始稳定性受到损害,有7例获得良好的初始稳定性,模型的平均微动范围在26.95~83.07μm之间,22.46%~86.97%假体柄多孔界面微动<40μm,此时可出现骨长入。结论为了获得良好的初始稳定性,假体柄与骨骼之间的完全接触不是必要的,一些接触率<100%的模型,包括近端接触和/或皮质接触,比全接触模型展现出更好的初始稳定性;假体柄的初始稳定性同时受到接触率及其位置的影响,在保证接触率>43%的情况下,假体柄应具有近端接触或者皮质骨接触,以保证获得良好的初始稳定性。Objective To explore the influence of stem-bone contact ratio and contact position of femoral prosthesis on initial stability after cementless hip arthroplasty by the finite element analysis.Methods Based on the DICOM CT image data of an adult volunteer femur,the solid models of femur and prosthesis stem were constructed by using Mimics and SolidWorks software.The solid models were positioned and assembled.The HyperMesh software was imported to divide the body mesh.The Mimics software was returned to use the CT gray value Assignment method to assign materials and establish the finite element model after hip arthroplasty.Finally,15 kinds of postoperative models with different shank-bone contact rates were set in ABAQUS software,and then stair climbing simulation analysis was carried out to calculate the fretting motion of 12171 nodes at the shankbone interface respectively.Results The initial stability was impaired in 8 of the 15 models,and good initial stability was achieved in 7 of the 15 models.The average fretting range of the model was 26.95~83.07μm,and the fretting of 22.46%~86.97%of the porous interface of the prosthetic stem was less than 40μm.Bone ingrowth could occur at this time.Conclusion In order to obtain good initial stability,complete contact between prosthesis stem and bone is not necessary.Some models with contact rate lower than 100%,including proximal contact and/or cortical contact,shows better initial stability than full contact models.The initial stability of the prosthetic stem is affected by the contact rate and its position.Under the condition that the contact rate is more than 43%,the prosthetic stem should have proximal contact or cortical bone contact to ensure good initial stability.
