摘  要:目的探讨结核分枝杆菌培养物滤液蛋白(MPB64)抗原免疫胶体金法检测在分枝杆菌快速鉴定中的应用价值。方法收集2018年7月至11月南宁市第四人民医院294例疑似结核病患者的分枝杆菌临床分离株,分别采用PNB生长试验和MPB64抗原免疫胶体金检测法进行菌种初步鉴定,以基因芯片法的菌株鉴定结果为比对标准。结果PNB生长试验和MPB64抗原免疫胶体金检测两种方法的符合率99.32%(292/294);PNB法的灵敏度99.63%(270/271),特异度95.83%(23/24);MPB64抗原胶体金检测法的灵敏度99.63%(269/270),特异度100%(24/24);两种分枝杆菌鉴定方法的检测结果无显著性差异(χ^2=0.091,P=0.763)。结论MPB64抗原免疫胶体金检测法操作更简便,特异度更高,能在培养阳性结果报告发出的当天即可做分枝杆菌鉴定并发出鉴定结果,较传统PNB培养法提前了28d。具有较高的临床应用价值。Objective To investigate the clinical application and utility value of MPB64 for colloidal gold immunochromatography technical detection in the rapid and accurate identification of mycobacterial species.Methods 294 clinical isolates from the genus Mycobacterium were collected from The Fourth People's Hospital of Nanning between July and November 2018.These isolates were identified via the growth trial and colloidal gold immunochromatography technology,and the results were verified by gene chip technology.Results The concordance rate between the growth trial and the colloidal gold immunochromatography technology was 99.32%(292/294).The sensitivity and specificity of the growth trial and the colloidal gold immunochromatography technology were 99.63%(270/271)and 195.83%(23/24),99.63%(269/270)and 100%(24/24),respectively.There was no significant difference in the identification rate between the two methods(χ^2=0.091,P=0.763).Conclusion The colloidal gold immunochromatography technology was easy to perform,specific and rapid testing with same day,which was 28 days in advance,and will be especially useful for clinical application than the PNB growth trial.
