摘  要:目的 探讨前交叉韧带重建术股骨侧定位点及移植物直径变化与膝关节旋转稳定性的相关性.方法 选取2014年1月~2019年3月我院收治的60例前交叉韧带损伤患者作为研究对象,对其实施前交叉韧带重建术,术后对其进行CT检查,分别进行如下2种实验.实验1中,确保移植物直径一致,对不同隧道定位点与膝关节旋转稳定性的相关性进行分析;实验2中,确保股骨侧定位点一致,根据实验不同的控制隧道直径进行分组,每组30例,在控制隧道直径为7 mm的情况下,根据不同距离分为a组(n=18)与A组(n=12);在控制隧道直径为8 mm的情况下,根据不同距离分为b组(n=13)与B组(n=17).在实验2中,将控制隧道直径为7 mm作为c组(n=30),将控制隧道直径为8 mm作为d组(n=30),对不同的隧道直径与膝关节旋转稳定性的相关性进行分析.在两组实验下观察股骨滑车沟距离(TT-TG值)的变化,并对患者的TT-TG值进行观察,分析两组实验中与膝关节旋转稳定性的相关性.结果 实验1中,在控制隧道直径为7 mm的情况下,a组的TT-TG值明显小于A组,差异有统计学意义(P<0.05);在控制隧道直径为8 mm的情况下,b组的TT-TG值明显小于B组,差异有统计学意义(P<0.05);相关性分析显示,定位点与膝关节旋转稳定性成正相关(P<0.05).实验2中,在定位点范围恒定情况下,c组与d组的TT-TG值差异无统计学意义(P>0.05);相关性分析显示,移植物直径变化与膝关节旋转稳定性无相关性(r=0.3617,P=0.0716).结论 前交叉韧带重建术移植物直径变化与膝关节旋转稳定性无相关性,但股骨侧定位点与膝关节旋转稳定性成正相关,因此需要加强对股骨侧定位点的重视,使其定点准确.Objective To investigate the correlation between the changes of femoral positioning point and graft diameter and the rotational stability of knee joint in anterior cruciate ligament reconstruction.Methods From January 2014 to March 2019,60 patients with anterior cruciate ligament injury in our hospital were selected as the study objects,anterior cruciate ligament reconstruction was performed,and CT examination was performed after operation.The following two experiments were carried out respectively.In experiment 1,ensuring the same graft diameter,the correlation between different tunnel positioning points and knee rotation stability was analyzed.In experiment 2,ensuring the femoral side positioning point was consistent,each group was divided into according to different control tunnel diameters,grouping was corried out,30 cases in each groups.When the control tunnel diameter was 7 mm,they were divided into group a(n=18)and group A(n=12)according to different distances.When the control tunnel diameter was 8 mm,they were divided into group b(n=13)and group B(n=17)according to different distances.In experiment 2,the control tunnel diameter of 7 mm were selected as group c(n=30).The control tunnel diameter of 8 mm were selected as group d(n=30).The correlation between the different tunnel diameter and the rotation stability of knee joint was analyzed.In the two groups of experiments,the change of the distance of trochlear groove(TT-TG value)was observed,and the TT-TG value of patients was observed,and the correlation between the two groups of experiments and the rotation stability of knee joint was analyzed.Results In experiment 1,the TT-TG value in group a was significantly lower than that in group A when the tunnel diameter was controlled to be 7 mm,the difference was statistically significant(P<0.05).In group b,the TT-TG value was significantly lower than that in group B when the tunnel diameter was controlled to be 8 mm,the difference was statistically significant(P<0.05).Correlation analysis showed that the posit
