摘 要:目的探讨近30余年健康成人肘关节X线角度的变化及其影响因素,为肘关节病变患者提供个性化诊疗依据。方法2016年12月~2017年12月期间招募健康青壮年102例进行双上肢DR摄片,测量双侧肘关节各角度值,与历史参考值进行对比,分析各角度与身高、体重及BMI的相关性,并分析各角度在不同侧别及性别之间是否存在统计学差异。结果1)肱轴角、前臂轴角的均值与历史均值之间存在统计学差异(P均<0.05);2)右侧前臂轴角与身高、右侧肱骨-肘-腕角与体重、右侧肱轴角与体重、右侧肱骨-肘-腕角与BMI、右侧肱轴角与BMI、左侧肱轴角与BMI呈弱相关(r/P=0.240/0.015、0.279/0.004、0.208/0.035、0.285/0.004、0.336/0.001、0.210/0.034);3)肱轴角及前臂轴角在左右侧之间存在统计学意义,t/P=3.435/0.001、2.458/0.016;右侧肱骨-肘-腕角及右侧前臂轴角在男女之间存在统计学差异,t/P=2.016/0.046、3.139/0.002。结论肘关节的肱轴角、前臂轴角度值与30年前的历史参考值存在统计学差异,并在侧别之间存在一定的差异;右侧肘关节各角度与身高、体重及BMI存在一定程度的相关性,右侧肘关节各角度在性别之间存在一定的差异。Objective To explore the regularity and influencing factors of X-ray Angle of elbow joint in healthy adults in recent 30 years,and to provide individualized diagnosis and treatment basis for elbow joint lesions.Methods From December 2016 to December 2017,102 healthy young and adults were recruited to take DR photographs of their upper limbs.The humerus-elbow-wrist Angle,humerus axis Angle and forearm axis Angle of bilateral elbow were measured and compared with historical reference values.The correlation between the angles and height,weight and BMI was analyzed,and whether there were statistical differences between the angles on different sides and between the sexes was analyzed.Results 1)There were statistical differences between the mean values of humerus axis Angle and forearm axis Angle and historical mean values(all P<0.05);2)There were weak correlations between right forearm axis Angle and height,right humerus-elbow-wrist Angle and body weight,righthumerus cois Angle and body weight,right humerus-elbow-wrist Angle and BMI,right humerus axis Angle and BMI,left humerus axis Angle and BMI(r/P=0.240/0.015,0.279/0.004,0.208/0.035,0.285/0.004,0.336/0.001,0.210/0.034);3)The humerus axis Angle and forearm axis Angle were statistically significant between the left and right sides,with t/P=3.435/0.001 and 2.458/0.016.There were statistically significant differences in the right humerus-elbow-wrist Angle and the right forearm axial Angle between men and women,with t/P=2.016/0.046 and 3.139/0.002.Conclusion There are statistically significant differences between the humeral axis and forearm axis angles of elbow joint and the historical reference values 30 years ago,and there are some differences between the sides;The Angle of the right elbow joint was weakly correlated with height,weight and BMI,and there are some differences between the Angle of right elbow joint and gender.However,cross-sectional survey data of multi-center,large sample size and whole population were still needed for further study.
