摘  要:本研究旨在用前臂外周定量计算机断层摄影术,评估婴幼儿的体积骨密度(vBMD),并研究其对婴幼儿佝偻病早期诊断的意义。选取2017年9月至2019年3月在本院的600例婴幼儿作为研究对象,所有研究对象记录姓名、性别、年龄、身高、体重,均应用骨密度检测仪测量胫骨中上1/3段的骨密度,同时检测血清中骨碱性磷酸酶浓度及维生素D3水平。以血清25(OH)D3<27.5 nmol/L,且符合佝偻病简易诊断标准初期的患儿作为诊断维生素D缺乏性佝偻病的标准,分为正常组和佝偻病组。两组年龄、身高、体重呈正态分布。两组骨密度值、骨碱性磷酸酶浓度用(x±s)表示,组间进行t检验。对于佝偻病组,分别计算骨密度的正确率和骨碱性磷酸酶的正确率,采用卡方检验,p<0.05为差异有统计学意义。正常组患儿骨密度高于佝偻病组,骨碱性磷酸酶低于佝偻病组;骨密度对于佝偻病的检出率高于碱性磷酸酶组,p<0.05具有统计学意义。The objective of this study was to evaluate the volume bone mineral density(VBMD)of infants and investigate its significance in the early diagnosis of infant rickets using peripheral quantitative computed tomography of the forearm.600 infants and young children in our hospital from September 2017 to March 2019 were selected as the research objects.All the objects were recorded name,gender,age,height,weight.Bone mineral density in the middle and upper third of the tibia was measured with a bone mineral density detector.The concentration of bone alkaline phosphatase and vitamin D3 levels in serum were also measured.Serum 25(OH)D3<27.5 nmol/L and children who met the initial criteria for the simple diagnosis of rickets were used as the standard for the diagnosis of vitamin D deficiency rickets,which was divided into the normal group and the rickets group.The age,height and weight of the two groups were normal distribution.Bone density and bone alkaline phosphatase concentration in two groups were used(mean plus or minus standard deviation),and t-test was performed between the groups.For rickets,the accuracy of bone density and bone alkaline phosphatase were calculated respectively,and chi-square test was used.p<0.05 was considered statistically significant.The bone density of the normal group was higher than that of rickets,and the bone alkaline phosphatase of the normal group was lower than the rickets group.The detection rate of bone mineral density for rickets was higher than that of alkaline phosphatase group.p<0.05 was considered statistically significant.
