腹腔镜阑尾切除术中改良丝线结扎法处理阑尾系膜及根部的临床研究

摘　　要：目的:研究腹腔镜阑尾切除术中采用改良丝线结扎法与单纯丝线结扎法处理阑尾系膜及根部的临床效果。方法:回顾分析2017年8月至2019年8月因急性阑尾炎行腹腔镜阑尾切除术的80例患者的临床资料,患者随机分为治疗组与对照组,每组40例。治疗组术中采用改良丝线结扎法处理阑尾系膜及根部,对照组术中采用单纯丝线结扎法,比较两组手术时间、术中出血量、术后肠功能恢复时间、术后早期下床时间、住院时间、住院总费用及术后发热、切口感染、腹腔积液、腹胀等并发症发生情况。结果:治疗组手术时间、术中出血量、术后肠功能恢复时间、术后腹胀发生率优于对照组,差异有统计学意义(P<0.05);两组术后早期下床时间、住院时间、住院总费用及术后发热、切口感染、腹腔积液等并发症发生率差异无统计学意义(P>0.05)。结论:腹腔镜阑尾切除术中采用改良丝线结扎法处理阑尾系膜及根部较单纯丝线结扎法具有一定优势,便于操作,利于临床开展。Objective:To compare the clinical effect of modified silk thread ligation in laparoscopic appendectomy for the treatment of mesentery and root of appendix with simple silk thread ligation.Methods:Clinical data of 80 patients with acute appendicitis who received laparoscopic appendectomy from Aug.2017 to Aug.2019 were retrospectively analyzed.The patients were randomly divided into treatment group and control group(40 cases in each group).The treatment group treated the mesentery and the root of appendix with modified silk thread ligation,while the control group treated the mesentery and the root of appendix with simple silk thread ligation.The operative time,the intraoperative bleeding volume,the time of recovery of intestinal function,the time of early postoperative ambulation,the hospital stay,the total cost of hospitalization,the postoperative fever,the infection of incision,the postoperative hydroperitoneum and the postoperative abdominal distension were compared between the two groups.Results:Compared with the control group,the treatment group had shorter operation time,less intraoperative blood loss,shorter recovery time of intestinal function,and lower incidence of postoperative abdominal distension(P<0.05).There were no significant differences in the time of early postoperative ambulation,the hospital stay,the total cost of hospitalization,the postoperative fever,the infection of incision or the postoperative hydroperitoneum between the two groups(P>0.05).Conclusions:Compared with simple silk thread ligation,modified silk thread ligation of appendix and root in laparoscopic appendectomy has certain advantages,is easy to operate and conducive to clinical development.
