珠海市某医院儿科住院患儿20年疾病谱变化规律研究

摘　　要：目的研究珠海市某三甲医院近20年住院患儿疾病谱及病死率的变化规律。方法选取珠海市某妇幼保健院1999年1月1日-2018年12月31日共137 849例出院病案资料,按患儿出院时间划分为1999年-2008年和2009年-2018年2个时间段组别对比研究。结果 20年间住院患儿住院平均天数6.77天,中位时间6天,其中男85669例,占62.2%。年龄集中于婴儿期(29天~1岁),占38.0%。在疾病系统构成中呼吸系统疾病居于首位,构成比42.1%,之后位次是新生儿疾病(27.1%)、传染病(9.5%)、消化系统疾病(7.4%)、血液系统疾病(3.5%)。死亡病例共127例,病死率呈逐年下降的趋势。死亡病例前5位疾病为新生儿肺炎、重症肺炎、新生儿败血症、新生儿重度窒息、败血症。结论儿科临床及预防工作的重点仍是呼吸系统疾病、新生儿疾病等防治工作,且要重视逐年增长的疾病。Objective To study the disease spectrum and mortality of hospitalized children in a third-grade class-A hospital in Zhuhai in recent 20 years.Methods The medical records of the children discharged from the hospital from January 1,1999 to December 31,2018 were selected as the research objects.According to the discharge time,the children were divided into two time groups:1999-2008 and 2009-2018.Results The average length of hospital stay was 6.77 days and the median time was 6 days.Including 85669 males,accounting for 62.2%.Most of them were in infancy(29 days to 1 year old),accounting for 38.0%.Respiratory system diseases always occupy the first place in the composition of disease system(42.1%).After that,the order was neonatal disease(27.1%),infectious disease(9.5%),digestive system disease(7.4%),blood system diseases(3.5%).The total number of deaths is 127,and the mortality rate has been decreasing year by year.The top five diseases of the dead children were neonatal pneumonia,severe pneumonia,neonatal sepsis,neonatal severe asphyxia and sepsis.Conclusion The focus of clinical and preventive work in pediatrics is still the prevention and treatment of respiratory system diseases and neonatal diseases,and we should pay attention to the growing diseases year by year.
