SBAR沟通模式下产科新生儿交接记录单的设计及应用

摘　　要：目的采用SBAR沟通模式对产科新生儿交接表进行设计、实践并评价效果。方法选取2017年2月1日-2019年9月30日期间转入某院新生儿重症监护室(新生儿病房)的107例患儿作为对照组,选取同期某院产科转入新生儿病房的108例患儿作为研究组。对照组采用传统产科和儿科交接,研究组则采用SBAR沟通下交接记录单,分析两组病情交接准确率及完整率、交接后不良事件发生率、交接耗时及交接满意度。结果研究组交接病情正确率99.1%,完整率100%,高于对照组的89.7%和87.8%,P=0.00;研究组交班不良事件发生率3.7%低于对照组12.1%,P=0.022;研究组交班耗时(12.9±5.1)分钟,短于对照组(16.2±5.0)分钟,P=0.00,研究组交班满意度97.7%高于对照组82.6%,P<0.05,两组各项指标比较,差异具有统计学意义。结论在产科和儿科新生儿交接时采用SBAR沟通下设计的交接记录单可提升病情交接时正确率及完整程度,规避交接后不良事件的发生,交接内容全面、规范,缩减交接时间,提升满意度,值得在产科和儿科中推广及深入研究。Objective Using SBAR communication mode to design,practice and evaluate the effect of obstetrical neonatal hand-in-hand table.Methods 107 children transferred to neonatal intensive care unit(neonatal ward)from February 1,2017 to September 30,2019 were selected as the control group,and 108 cases transferred to the neonatal ward in the obstetrics department of a hospital in the same period were selected as the study group.The control group used traditional obstetrics and pediatrics handover,while the study group used SBAR handover record sheet.The accuracy and integrity of handover,the incidence of adverse events after handover,handover time and handover satisfaction of the two groups were analyzed.Results The correct rate of handover in the study group was 99.1%,and the integrity rate was 100%,which was higher than 89.7%and 87.8%in the control group,P=0.00.The incidence of shift adverse events in the study group was 3.7%lower than that in the control group(12.1%),P=0.022;The shift time in the study group was(12.9±5.1)minutes,which was shorter than that in the control group(16.2±5.0)minutes,P=0.00.The shift satisfaction of the study group was 97.7%higher than that of the control group 82.6%(P<0.05).The difference between the two groups was statistically significant.Conclusion Using the handover record sheet designed under SBAR communication during neonatal handover in obstetrics and pediatrics can improve the accuracy and integrity of the handover,avoid the occurrence of adverse events after handover,make the handover content comprehensive and standardized,shorten the handover time,and improve satisfaction,which is worthy of popularization and in-depth study in obstetrics and pediatrics.
