摘　　要：目的通过比较某院维持性血液透析死亡与存活患者去脂体重指数及各临床指标的不同,明确其死亡与去脂体重指数及各临床指标的关系,并探索低去脂体重指数对血液透析患者死亡的预测价值。方法分析2017年4月-2019年7月于某院规律血液透析,自愿参加人体成分分析且保留完整数据患者153例,分为死亡组(23人)和存活组(130人),比较两组患者与去脂体重指数及其他临床指标的相关性。结果单因素危险因素分析发现,血液透析患者在低去脂体重指数、年龄、BNP、CRP、血白蛋白、血胆固醇、低密度脂蛋白水平较存活组的差异均有统计学意义(P<0.05)。Logistic回归分析提示,血液透析患者低去脂体重指数与死亡呈正相关(OR=5.953,P<0.05);患者年龄与死亡呈正相关(OR=1.069,P<0.05)。结论血液透析患者,去脂体重指数越低,年龄越大,其死亡风险越高。低去脂体重指数对预估血液透析患者的死亡有一定的参考价值,从而为临床更好地制定医疗保健策略提供可能。Objective To compare the difference of fat free mass indexand clinical indicators between the death and survival patients of Maintenance Hemodialysisin our hospital,to clarify the relationship between the death and FFMI and clinical indicators,and to explore the predictive value of low FFMI for the death of MHD patients.Methods 153 patients with regular hemodialysis from April 2017 to July 2019 who volunteered to participate in human body composition analysis and retained complete data were divided into death group and survival group.The correlation between the two groups and FFMI and other clinical indicators was compared.Results Univariate risk factor analysis showed that the levels of low FFMI,age,BNP,CRP,serum albumin,serum cholesterol and low density lipoprotein in MHD patients were significantly different from those in survival group(P<0.05).Logistic regression analysis showed that low FFMI was positively correlated with mortality in MHD patients(OR=5.953,P<0.05);age was positively correlated with mortality(OR=1.069,P<0.05).Conclusion The lower the FFMI is,the older the age is,the higher the risk of death is.Low FFMI has certain reference value in predicting the mortality of MHD patients,thus providing the possibility for better formulation of health care strategies in clinic.
