氨氯地平联合阿托伐他汀对老年高血压合并高脂血症患者氧化应激及脂肪因子的影响

摘　　要：目的分析氨氯地平联合阿托伐他汀对老年高血压合并高脂血症患者氧化应激及脂肪因子的影响。方法选取2018年5月-2019年5月江西省南丰县中医院收治的老年高血压合并高脂血症患者126例,采用随机数字表法分为观察组与对照组各63例。2组均进行常规干预,对照组使用氨氯地平治疗,观察组采用氨氯地平联合阿托伐他汀治疗。比较2组临床疗效、治疗前后血压、血脂、氧化应激指标及脂肪因子水平。结果治疗后2组收缩压(SBP)、舒张压(DBP)、低密度脂蛋白胆固醇(LDL-C)、总胆固醇(TC)及三酰甘油(TG)水平均低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。观察组总有效率为95.2%,高于对照组的82.5%(χ^2=6.921,P<0.01)。治疗后,2组谷胱甘肽过氧化物酶(GSH-Px)及超氧化物歧化酶(SOD)水平均高于治疗前(P<0.05),丙二醛(MDA)水平低于治疗前(P<0.05),且观察组变化幅度大于对照组(P<0.05)。治疗后,2组瘦素水平均低于治疗前(P<0.05),脂联素及Vaspin水平高于治疗前(P<0.05),且观察组变化幅度大于对照组(P<0.05)。结论氨氯地平联合阿托伐他汀治疗老年高血压合并高血脂的效果较好,促使血压及血脂指标恢复正常,改善患者体内的氧化应激指标因子及脂肪因子水平。Objective To analyze the effects of amlodipine combined with atorvastatin on oxidative stress and adipokine in elderly patients with hypertension and hyperlipidemia.Methods 126 elderly patients with hypertension and hyperlipidemia admitted to Nanfeng County Hospital of traditional Chinese medicine from May 2018 to may 2019 were selected and randomly divided into observation group and control group with 63 cases in each group.Both groups received routine intervention,the control group was treated with amlodipine,and the observation group was treated with amlodipine combined with atorvastatin.The clinical effect,blood pressure,blood lipid,oxidative stress index and fat factor level before and after treatment were compared between the two groups.Results After treatment,the levels of systolic blood pressure(SBP),diastolic blood pressure(DBP),low density lipoprotein-cholesterol(LDL-C),total cholesterol(TC)and triacylglycerol(TG)were lower than before treatment(P<0.05),and the observation group was lower than the control group(P<0.05).The total effective rate of the observation group was 95.2%,higher than 82.5%in the control group(X^2=6.921,P<0.01).After treatment,the levels of glutathione peroxidase(GSH-Px),superoxide dismutase(SOD),adiponectin,and Vaspin were higher than those before treatment(P<0.05),MDA,the level of leptin was lower than before treatment(P<0.05),and the change of the observation group was greater than that of the control group(P<0.05).Conclusion Amlodipine combined with atorvastatin has a good effect on the treatment of elderly hypertension with hyperlipidemia,promotes the return of blood pressure and blood lipid indexes to normal,and improves the level of oxidative stress index factors and adipokine in patients.
