某县级医院前置审方开展前门急诊处方问题分析与干预

摘　　要：目的了解如皋市人民医院门急诊处方常见不合理处方问题,通过与临床医师沟通反馈、向临床医师培训宣教及医务部门绩效考核等方式进行干预,以期提高医师开具处方的合理率。方法通过PASS合理用药系统,收集2018年1-12月如皋市人民医院的门急诊处方610 149张,对存在问题的处方进行统计分析,对主要科室不合理处方的类型进行分类汇总,并进行6个月的干预,比较干预前后处方合理率。结果 2018年1-12月该院门急诊药房调配处方610 149张,点评不合理处方57 504张,占处方总数的9. 42%,其中不规范处方14 170张(24. 64%),不适宜处方41 643张(72. 42%),超常处方1 691张(2. 94%),处方合理率90. 58%;收集2019年1-6月我院的门急诊处方370 826张,采用同样的方法进行点评,对不合理处方进行6个月的干预后,处方合理率由2018年的90. 58%升高至2019年的97. 19%。结论医院门急诊处方合理率已达97%以上,今后仍需继续推行处方质量的持续改进,力争为正式开展处方前置审核排除障碍,为切实落实《医疗机构处方审核规范》打好基础。Objective To understand the common unreasonable prescription problems of outpatient and emergency prescriptions in Rugao People’s Hospital,and to intervene through communication with clinicians,training and education of clinicians,and performance evaluation of medical departments,in order to increase the reasonable rate of prescriptions issued by doctors. Methods Through PASS rational medication system,outpatient and emergency prescriptions( 610 149) from Rugao People’s Hospital from January to December 2018 were collected,statistically analyzed the prescriptions in question,and the types of unreasonable prescriptions in major departments were categorized and summarized,and 6 months of intervention,and the reasonable rate of prescription before and after intervention were compared. Results 610 149 prescriptions were dispatched from the outpatient emergency pharmacy of the hospital From January to December 2018,57 504 prescriptions were unreasonably reviewed,accounting for 9. 42% of the total prescriptions. Of these,14 170 were irregular prescriptions( 24. 64%)and 41 643 were not suitable( 72. 42%),over 1 691 prescriptions( 2. 94%),the reasonable rate of prescriptions was 90. 58%;After collecting 370 826 outpatient and emergency prescriptions in our hospital from January to June 2019,and using the same method to comment,after 6 months of intervention on unreasonable prescriptions,The reasonable rate of prescriptions in this hospital increased from 90. 58% in 2018 to 97. 19% in 2019. Conclusion The reasonable rate of prescriptions in the outpatient emergency department of the hospital has reached more than 97%,in the future,it is still necessary to continue to promote the continuous improvement of prescription quality,and strive to eliminate the obstacles for the formal pre-formulation review and lay a solid foundation for the implementation of the"Regulations for Prescription Review of Medical Institutions".
