摘 要:目的探讨品管圈(QCC)在提高急诊抗菌药物合理使用中的应用效果。方法成立QCC,按照QCC活动步骤,随机抽取改善前2019年2月上海市浦东新区人民医院6060张急诊抗菌药物处方进行统计分析,运用质量管理工具对存在问题进行改进;再次随机抽取改善后2019年5月上海市浦东新区人民医院急诊抗菌药物处方进行统计分析,比较实施QCC活动前后急诊抗菌药物合理使用率变化,由82.18%提升至92.27%。结果通过美康系统软件随机抽取2019年5月急诊抗菌药物处方共7500张,急诊抗菌药物使用存在选药不合理(1.63%)、联用不适宜(1.31%)、用法用量不适宜(1.17%)、诊断与用药不符(1.23%)、选药级别过高(1.12%)、无指征使用(0.75%)及重复用药(0.53%)等7类问题,共580张,抗菌药物使用合理率明显高于实施前,差异有统计学意义(P<0.05)。结论QCC活动不仅提高了急诊抗菌药物合理使用率,而且使来自不同部门的圈员们的工作积极性、创造性及协作性均得以提升。Objective To explore the application effects of quality control circle in improving rational use of antibiotics in emergency.Methods A quality control circle was established,and according to the activity steps of the quality control circle,6060 emergency antimicrobial prescriptions of Shanghai Pudong New Area People’s Hospital in February 2019 before improvement were randomly selected for statistical analysis.Quality management tools were used to improve existing problems.The prescriptions of emergency antibiotics in Shanghai Pudong New Area People’s Hospital in May 2019 after the improvement were randomly selected again for statistical analysis,and the changes in the reasonable utilization rate of emergency antibiotics before and after the improvement were compared,which increased from 82.18%to 92.27%.Results A total of 7500 emergency antimicrobial prescriptions were randomly selected through Maykang system software.The use of emergency antimicrobial drugs was unreasonable(1.63%),inappropriate combined use(1.31%),and inappropriate usage and dosage(1.17%),diagnosis and medication not matching(1.23%),too high level of drug selection(1.12%),use without pointer(0.75%)and repeated medication(0.53%),etc.in a total of 580 prescriptions.A reasonable rate of antimicrobial use was more obvious than that before implementation,and the difference was statistically significant(P<0.05).Conclusion The quality control circle approach has achieved good results in improving the rational use of antibiotics in emergency.Moreover,the work enthusiasm,creativity and collaboration of circle members from different departments can be improved.
