重症单元危重型新型冠状病毒肺炎患者死亡病例临床特征及原因分析

摘　　要：2020年1月13日—3月31日武汉大学人民医院东院重症医学科(重症单元)共收治COVID-19患者25例,其中死亡5例,采集5例患者的流行病学史、发病经过、临床症状及体征、实验室检查、影像学表现并进行死因分析。结果发现,5例危重型COVID-19患者中,3例有既往基础疾病,临床表现典型,影像学检查示肺部损伤严重。均给予抗生素、抗病毒、糖皮质激素、免疫调理、氧疗等规范治疗,其中1例应用ECMO。5例患者均死于急性呼吸窘迫综合征(ARDS)、呼吸衰竭,发病至死亡时间(21.80±4.78)d。Since coronavirus disease 2019(COVID-19)have breakout in the whole world in December 2019,many COVID-19 patients have been admitted to hospitals all over the world.The spreading speed and scale of the novel coronavirus has never been seen before,and is mainly manifested in the tendency of family aggregation.It can be transmitted through direct contact,droplet and aerosol transmission.From January 13th to March 10th in 2020,16 COVID-19 patients were admitted to the ICU department,among which 5 death cases were analyzed,discussed and summarized.The epidemiological history,course of onset,clinical symptoms and signs,laboratory examination,imaging manifestations of all patients were collected and the above results were analyzed retrospectively.Severe COVID-19 patients are mostly died of ARDS and respiratory failure.The 2019 new coronavirus affected multiple organs in the body.It is necessary to give timely antibiotics,antiviral,respiratory support,nutritional support and maintain the balance of water electrolytes,and careful nursing.These measures can effectively control the death rate of 2019 novel coronavirus pneumonia.
