摘  要:目的观察体外受精-胚胎移植(IVF-ET)周期中2、4细胞卵裂球多核细胞的发育情况,并观察其胚胎移植后的妊娠结局。方法选取行IVF-ET的患者96例,人工受精后时差成像技术观察胚胎卵裂球发育情况,将其分为均无多核胚胎(A组)、2细胞卵裂球多核者(2细胞一个卵裂球多核及4细胞无多核、2细胞均有多核及4细胞无多核胚胎,B组)、4细胞卵裂球多核者(2细胞一个卵裂球多核及4细胞至少一个卵裂球多核、2细胞均有多核及4细胞至少一个卵裂球多核胚胎,C组)、2细胞无多核且4细胞至少一个卵裂球多核胚胎者(D组)。结果A组培养第3天优质胚胎率和培养第5、6天可利用囊胚率均显著高于B、C、D组(P均<0.05),B组培养第3天优质胚胎率与培养第5、6天可利用囊胚率均优于C组(P均<0.05)。在妊娠结局方面,A组行72个周期胚胎移植,种植率58.33%(42/72),流产率7.14%(3/42),活产率54.17%(39/72);B组共行8个周期胚胎移植,其中2细胞一个卵裂球多核及4细胞无多核胚胎4个,3例患者种植成功,0例流产,3例均为活产;2细胞均有多核及4细胞无多核胚胎4个,4例患者种植成功,1例流产,3例活产。结论2、4细胞卵裂球多核均可影响胚胎发育及移植后妊娠结局,其中4细胞卵裂球多核对胚胎发育影响更明显。Objective We observed the development of 2-cell/4-cell blastomere multinucleate cells during the in vitro fertilization and embryo transfer(IVF-ET)cycle and the pregnancy outcome after embryo transplantation.Methods We selected 96 patients undergoing IVF-ET,and their embryos were dynamically observed by time-lapse imaging(TLI)(n=613).The embryos were divided into the non-multinuclear embryos(group A),2-cell embryos with multinucleus(2-cell embryos with one blastomere multinucleus and 4-cell embryos without multinucleus,2-cell embryos with multinucleus and 4-cell embryos without multinucleus,group B),4-cell embryos with multinucleus(2-cell embryo with one blastomere multinucleus and 4-cell embryos with at least one blastomere multinucleus,2-cell embryos with multinucleus and 4-cell embryos with at least one blastomere multinucleus,group C),2-cell embryos without multinucleus and 4-cell embryos with at least one blastomere multinucleus(group D).Results The rate of high-quality embryos on day 3 and the available blastocysts on day 5 and 6 in the group A were significantly higher than those in the groups B,C,and D(all P<0.05).The high-quality embryo rate on day 3 and the available blastocyst rate on day 5 and 6 in the group B were better than those in the group C(all P<0.05).In terms of pregnancy outcomes,patients in the group A underwent 72 cycles of embryo transfer,with the implantation rate of 58.33%(42/72),the abortion rate of 7.14%(3/42),and the live birth rate of 54.17%(39/72);a total of 8 cycles of embryo transfer were performed in the group B,there were 4 embryos of 2-cell embryos with one blastomere multinucleus and 4-cell embryos without multinucleus,3 patients were successfully implanted,with 0 abortion,and 3 cases of live births;there are 4 embryos of 2-cell embryos with multinucleus and 4-cell embryos without multinucleus,4 patients were successfully implanted,with 1 case of abortion,3 cases of live births.Conclusion Both 2-cell/4-cell blastomere multinucleate cells may affect the embryonic development p
