基于生态位构建的环渤海医疗服务圈发展建设研究

摘　　要：目的通过研究环渤海及其毗邻区域医疗服务水平差异,为该区域医疗发展格局构建和协同提供决策依据。方法运用生态位相关理论,通过极差变换法和熵值法对环渤海及其毗邻区域8个省市自治区医疗发展生态位进行综合评价。结果样本区域医疗服务圈评价指标权重居前3位的是住院服务水平(权重0.127)、地区经济水平(权重0.110)和社会发展水平(权重0.062),而样本区域医疗服务发展生态位次居前3位的是北京(生态位0.147)、天津(生态位0.101)和辽宁(生态位0.059)。同时,根据生态位次划分为环渤海区域医疗发展外环圈、中环圈、内环圈,即外环圈包括河北(生态位0.048)、河南(生态位0.047)、内蒙古(生态位0.044)和山西(生态位0.027),中环圈包括辽宁(生态位0.059)、山东(生态位0.058),内环圈包括北京(生态位0.147)、天津(生态位0.101)。住院服务水平在区域医疗服务圈生态位次上影响较大,且京津区域位于该医疗服务圈核心。结论应构建医疗生态位格局,疏解北京非首都功能,促进医疗输入与输出,推进环渤海区域协同发展。Objective To provide decision-making basis for the pattern construction and collaboration development of region medical by studying the difference of medical service level between the Bohai Rim and its adjacent areas.Methods Using niche theory,the niche of medical development in 8 provinces and autonomous regions around the Bohai Rim and its adjacent areas was evaluated comprehensively by range transformation method and entropy method.Results The top 3 weights of service circle evaluation indicators were hospitalization service level(0.127),regional economic level(0.110) and social development level(0.062),while the top 3 ecological niches of medical service development in sample areas were Beijing(0.147),Tianjin(0.101),Liaoning(0.059).At the same time,according to the niche potential,the sample areas could be divided into outer ring,middle ring and inner ring.The outer ring included Hebei(0.048),Henan(0.047),Inner Mongolia(0.044),Shanxi(0.027),middle ring included Liaoning(0.059),Shandong(0.058),and inner ring included Beijing(0.047) and Tianjin(0.101).The level of inpatient service had a great influence on the ecological potential of the regional medical service circle,and Beijing-Tianjin region was located in the core of the medical service circle.Conclusion The medical niche pattern should be constructed to relieve Beijing’s non-capital function,promote medical input and output,and promote the coordinated development of the Bohai Rim.
