同种异体骨治疗膝邻近关节骨内腱鞘囊肿效果观察

摘　　要：目的观察同种异体骨在膝邻关节骨内腱鞘囊肿中植骨应用的效果。方法 23例膝邻关节骨内腱鞘囊肿患者中股骨远端7例,胫骨近端15例,髌骨1例。采用囊肿刮除、同种异体骨植骨手术。术前术后随访,采用疼痛视觉模拟评分(VAS)评估患者疼痛缓解情况,膝关节活动Lysholm评分评估膝关节活动情况,Mankin评分评估同种骨移植植骨融合结果。结果所有患者随访2~3年,疼痛视觉模拟评分,术前(4.65±1.80)分,术后3个月(1.65±1.07)分,手术前后比较差异有统计学意义(P<0.05),末次随访(1.34±1.23)分。膝关节活动受限患者10例,术前膝关节活动评分(62.80±4.63)分,术后(80.90±4.20)分,手术前后Lysholm评分比较差异有统计学意义(P<0.05),Mankin评分评估同种骨移植植骨融合结果均达到满意效果。无囊肿复发者及并发症发生者。结论膝邻关节骨内腱鞘囊肿在病灶刮除后,采用同种异体骨植骨可获满意的效果。Objective To observe the effect of allogeneic bone on bone grafting in sacral sheath cysts of knee joints.Methods 23 patients with patella femoral cysts of the knee joints were included in the distal femur,7 in the proximal femur,15 in the proximal humerus and one in the tibia.Surgery was performed with cyst scraping and allogeneic bone grafting.Preoperative and postoperative following-up of patients was performed,pain relief was assessed by using visual analogue scale(VAS),knee joint activity Lysholm score was used to evaluate knee joint activity,and Mankin score was used to evaluate bone graft fusion results.Results All patients were followed up for 2 to 3 years,pain VAS were 4.65±1.80 points in preoperative,and 1.65±1.07 points in postoperative 3 months.The difference was statistically significant before and after surgery(P<0.05).And the last followed-up of pain VAS was 1.34±1.23 points.There were 10 patients with limited knee joint activity,and scores in preoperative knee activity was 62.80±4.64 and scores in postoperative was 0.90±4.20;The Lysholm scores was statistically significant before and after surgery(P<0.05).Mankin scores were achieved satisfactory results in bone graft fusion.No cyst recurrence and complications occurred.Conclusion The iliac sheath cyst of the adjacent joint of the knee joint can be treated well with the same kind of bone graft after lesion.
