小网片在手术治疗老年女性盆腔器官脱垂中的临床应用

摘　　要：目的探讨临床应用经阴道植入小网片治疗保留子宫的老年性盆腔器官脱垂的可行性及临床效果。方法回顾性分析86例盆腔器官脱垂经手术治疗患者的临床资料,所有患者均予经阴道植入网片行前盆腔悬吊术,根据患者意愿及手术指征分为保留子宫组(保宫组)、经阴道切除子宫组(切宫组)。结果保宫组患者手术耗时、术中出血量、术后住院时间及住院费用均较切宫组患者明显缩短,差异有统计学意义(P<0.05);保宫组患者经阴道前壁植入小网片治疗术后围手术期并发症少于切宫组患者;术后随访1年2组患者未见小网片暴露。结论小网片治疗盆腔器官脱垂手术时间及术后住院时间短,复发率低。Objective To explore the feasibility and clinical effect of transvaginal implantation of small mesh in the treatment of senile pelvic organ prolapse with uterine retention.Methods 86 patients undergoing surgical treatment of pelvic organ prolapse were retrospectively analyzed.All patients were given anterior pelvic suspension through vaginal implantation.They were randomly divided into the study group and control group according to the patient's wishes and surgical indication.Results The patients two groups were followed up for one year after surgical treatment.Compared with the clinical application results,the surgical time,intraoperative blood loss,postoperative hospitalization time and hospitalization cost of patients with uterine-sparing surgery were significantly shorter than those in patients with hysterectomy(P<0.05).Compared with the uterine resection group,the uterine preservation group with small meshes implanted had lower perioperative complications.No small meshes were exposed after surgery in the two groups.Conclusion Anterior pelvic suspension with uterine preservation for the treatment of pelvic organ prolapse in elderly women has a short operation time and postoperative hospital stay and with lower recurrence rate.
