超声对小儿回-结肠套叠的诊断价值

摘　　要：目的探讨超声对小儿回-结肠套叠的诊断价值。方法选择2018年8月至2019年1月于我院就诊的118例超声检查疑似回-结肠套叠患儿作为研究对象,所有患儿均经超声检查发现同心圆征,分析检查结果。结果经X线空气灌肠未见肠套叠征象者17例,占14.41%;未经X线空气灌肠仅服药观察未见异常者9例,占7.63%。118例患儿经超声提示同心圆征位于右侧腹者105例,经X线空气灌肠证实87例为肠套叠,符合率为82.86%;位于左侧腹者13例,经证实3例为肠套叠,符合率为23.08%;超声诊断右侧腹的符合率高于左侧腹,差异有统计学意义(P <0.01)。118例患儿中同心圆包块直径>30 mm者90例,均被证实为肠套叠,符合率为100.00%;直径<30 mm者28例,经证实仅2例为肠套叠,符合率为7.14%;超声诊断同心圆包块直径>30 mm的符合率高于直径<30 mm,差异有统计学意义(P <0.01)。结论超声诊断小儿回-结肠套叠具有较高的符合率,可为临床诊治提供重要依据。Objective To explore the value of ultrasound in the diagnosis of pediatric ileum-colon intussusception.Methods 118 children with suspected ileum-colon intussusception examined by ultrasound from August 2018 to January 2019 in our hospital were selected as study objects.All cases were found with concentric circle sign by ultrasound examination,and the examination results were analyzed.Results X-ray air enema showed 17 cases(14.41%)without intussusception signs,and 9 cases(7.63%)only received medicine and observation without X-ray air enema.Among 118 children,105 cases had concentric circle sign on the right abdomen showed by ultrasound,among them 87 cases were confirmed as intussusception by X-ray air enema,with the coincidence rate of 82.86%;13 cases had concentric circle sign on the left abdomen,among them 3 cases were confirmed as intussusception by X-ray air enema,with the coincidence rate of 23.08%;The coincidence rate of right abdomen diagnosed by ultrasound was higher than that of left abdomen,with statistically significant difference(P<0.01).Among 118 children,90 cases with diameter of concentric circle mass>30 mm were all confirmed as intussusception,with the coincidence rate of 100.00%;In 28 cases with diameter<30 mm,only 2 cases were confirmed as intussusception,with the coincidence rate of 7.14%;The coincidence rate of diameter of concentric circle mass>30 mm diagnosed by ultrasound was higher than that of diameter<30 mm,with statistically significant difference(P<0.01).Conclusions Ultrasound in the diagnosis of pediatric ileum-colon intussusception has high coincidence rate,which can provide important basis for clinical diagnosis and treatment.
