摘　　要：目的探讨女性和男性年龄对宫腔内人工授精(IUI)助孕妊娠结局的影响。方法收集2018年3月至2019年12月共2 424个IUI周期的临床资料,按照女性年龄分为A组(≤34岁)、 B组(35~39岁)、 C组(≥40岁),按男性年龄分为D组(≤34岁)、 E组(35~39岁)、 F组(≥40岁)。比较各组的临床妊娠率、生化妊娠率和早期自然流产率。结果 A、 B、 C组的临床妊娠率分别为12.48%、 9.07%、 5.76%;A组的临床妊娠率显著高于B组、 C组(P <0.05);C组的生化妊娠率为7.91%,显著高于A组的3.37%、 B组的2.53%(P <0.05);A、 B、 C组的早期自然流产率分别为8.41%、 30.23%、 12.50%;A组的早期自然流产率显著低于B组(P <0.05)。D、 E、 F组的临床妊娠率分别为12.47%、 10.70%、 8.19%;F组的临床妊娠率显著低于D组(P <0.05);D、 E、 F组的生化妊娠率比较无统计学差异(P>0.05);D、 E、 F组的早期自然流产率分别为8.11%、 20.31%、17.86%;E组的早期自然流产率显著高于D组(P <0.05)。结论当女性和/或男性年龄≥40岁时,IUI妊娠结局较差,应考虑更为积极的助孕手段。Objective To explore the influence of male and female age on the pregnancy outcome of intrauterine insemination(IUI).Methods The clinical data of 2424 IUI cycles from March 2018 to December 2019 were divided into group A(≤34 years old),group B(35 to 39 years old)and group C(≥40 years old)according to female age,and were divided into group D(≤34 years old),group E(35 to 39 years old)and group F(≥40 years old)according to male age.The clinical pregnancy rate,biochemical pregnancy rate and early spontaneous abortion rate of each group were compared.Results The clinical pregnancy rates of group A,group B and group C were 12.48%,9.07%and 5.76%,respectively.The clinical pregnancy rate of group A was significantly higher than that of group B and group C(P<0.05);The biochemical pregnancy rate of group C was 7.91%,significantly higher than 3.37%of group A and 2.53%of group B(P<0.05);The early spontaneous abortion rates of group A,group B and group C were 8.41%,30.23%and 12.50%,respectively.The early spontaneous abortion rate of group A was significantly lower than that of group B(P<0.05).The clinical pregnancy rates of group D,group E and group F were 12.47%,10.70%and 8.19%,respectively.The clinical pregnancy rate of group F was significantly lower than that of group D(P<0.05);No statistical difference was found in the biochemical pregnancy rate among group D,group E and group F(P>0.05);The early spontaneous abortion rates of group D,group E and group F were 8.11%,20.31%and 17.86%,respectively.The early spontaneous abortion rate of group E was significantly higher than that of group D(P<0.05).Conclusions The IUI pregnancy outcome is worse if female and/or male age is more than 40,and positive assisted reproductive technology should be considered.
