摘 要:目的探讨抗体筛查联合吸收放散试验在疑难配血中的应用效果。方法选取2017年1月至2019年6月我院进行疑难配血的60例患者,随机分为两组各30例。对照组进行抗体筛查,观察组采用抗体筛查联合吸收放散试验,比较两组的疑难配血情况、疑难配血原因分布情况。结果疑难配血原因中,不规则抗体、自身抗体、 ABO血型抗原减弱及ABO亚型占比较高,分别为40.00%、 30.00%、 15.00%、 5.00%,其他合计占比为10.00%;观察组的正反定型符合率为90.00%,高于对照组的63.33%(P <0.05)。结论临床疑难配血原因有多种,根据原因采取对应措施,通过抗体筛查可排除干扰因素,灵活运用吸收放散实验可用于检测较弱的抗原及抗体,保证患者输血的安全有效。Objective To explore the application effect of antibody screening combined with absorption and release test in difficult blood matching.Methods 60 patients with difficult blood matching in our hospital from January 2017 to September 2019 were selected and randomly divided into two groups,with 30 cases in each group.The control group received antibody screening,and the observation group received antibody screening combined with absorption and release test.The difficult blood matching and the distribution of difficult blood matching causes were compared between the two groups.Results In the distribution of difficult blood matching causes,the irregular antibody,autoantibody,decrease of ABO blood group antigen and ABO subtype accounted for 40.00%,30.00%,15.00%and 5.00%,and the other total proportion was 10.00%.The coincidence rate of positive and negative stereotypes in the observation group was 90.00%,higher than 63.33%in the control group(P<0.05).Conclusions There are many reasons for clinical difficult blood matching.Corresponding measures should be taken to eliminate the interference factors through antibody screening according to the reasons.The absorption and release test can be used to detect weaker antigens and antibodies to ensure the safety and efficacy of blood transfusion.
