针刺配合盆底肌训练治疗产后早期压力性尿失禁的疗效观察

摘　　要：目的观察针刺配合盆底肌训练治疗产后早期压力性尿失禁的临床疗效。方法将100例产后早期压力性尿失禁患者随机分为治疗组和对照组,每组50例。治疗组采用针刺配合盆底肌训练治疗,对照组采用单纯盆底肌训练治疗。治疗8周后,观察两组治疗后尿垫试验阳性率、尿垫试验评分、盆底肌肌力值、盆底肌肌疲劳度值、国际尿失禁咨询委员会尿失禁问卷表简表(ICI-Q-SF)评分及生活质量量表(SF-36)各项评分的变化情况,并比较两组临床疗效。结果治疗组总有效率为94.0%,对照组为74.0%,两组比较差异具有统计学意义(P<0.05)。治疗组治疗后尿垫试验阳性率及评分、盆底肌肌力值和肌疲劳度值各项指标、ICI-Q-SF评分及SF-36各项评分与对照组比较,差异均具有统计学意义(P<0.05)。结论针刺配合盆底肌训练是一种治疗产后早期压力性尿失禁的有效方法,能改善盆底肌功能。Objective To observe the clinical efficacy of acupuncture plus pelvic floor muscle exercise in treating early-stage postpartum stress urinary incontinence(SUI).Method One hundred patients with early-stage postpartum SUI were randomized into a treatment group and a control group,with 50 cases in each group.The treatment group was intervened by acupuncture plus pelvic floor muscle exercise,while the control group was intervened by pelvic floor muscle exercise alone.After 8-week treatment,changes in the positive pad test rate,pad test score,pelvic floor muscle strength,pelvic floor muscle fatigue value,and the scores of International Consultation on Incontinence Questionnaire-Urinary Incontinence-Short Form(ICIQ-UI-SF)and 36-item Short-Form Health Survey(SF-36)were observed in the two groups.The clinical efficacy was also compared between the two groups.Result The total effective rate was 94.0%in the treatment group versus 74.0%in the control group,and the between-group difference was statistically significant(P<0.05).After treatment,the positive rate and score of pad test,pelvic floor muscle strength and fatigue value,and the scores of ICIQ-UI-SF and SF-36 in the treatment group were significantly different from those in the control group(P<0.05).Conclusion Acupuncture plus pelvic floor muscle exercise is an effective approach for early-stage postpartum SUI,and it can improve the pelvic floor muscle function.
