开颅术后颅内感染患者药学会诊的病例回顾分析

摘　　要：目的:探讨分析我院开颅术后颅内感染(post-craniotomy intracranial infection,PCII)患者的疾病特点、会诊意见,为提升临床药师会诊水平提供经验。方法:采用回顾性研究的方法,调取2014年1月-2019年12月该院临床药师参与PCII患者会诊的病历,对病例特点和会诊意见进行统计分析。结果:医师申请会诊时55.63%患者发烧、59.73%意识障碍、60.07%合并其他部位感染、69.62%抗感染药物使用超过8 d,且66.55%至少使用2种;会诊目的39.93%是请药师协助解决PCII合并感染、14.33%为制定泛耐药菌用药方案、31.06%是处理不明原因发热;293例临床药师会诊意见中255例被完全采纳,采纳率87.30%,完全采纳后的治疗有效率为95.29%。结论:PCII患者疾病复杂,临床药师需结合会诊时机分析医师申请会诊目的,制定针对性专业药学意见,协助医师提高PCII患者治愈率。Objective:To retrospectively analyze the disease characteristics and consultation suggestions of patients with postcraniotomy intracranial infection(PCII)in the department of neurosurgery,in order to provide experience for pharmacist consultation.Methods:Data of patients with PCII consulted by clinical pharmacists from January 2014 to December 2019 were retrospectively collected.And the case characteristics and consultation suggestions were statistically analyzed.Results:Among the enrolled patients,55.63%of the patients had fever,59.73%had consciousness,60.07%were combined with other infection,69.62%had used anti-infective drugs for more than 8 days and 66.55%used two or more anti-infective drugs at the same time when applying for pharmaceutical consultation.39.93%consultations were applied for PCII,14.33%were for making regimen of pan-resistant organism and 31.06%were for fever of unknown origin.255 of the 293 opinions from clinical pharmacists were fully adopted,which was accounted for 87.30%.And the effective rate of treatment after adopting consultation suggestions was 95.29%.Conclusion:Considering that the condition of patients with PCII was very complicated,clinical pharmacists should analyze the purpose of the consultation combined with the applying time,put forward target professional pharmaceutical opinions in order to improve the cure rate of PCII patients.
