NLRP3炎症体分子与AECOPD患者的相关性研究

摘　　要：[目的]探讨NLRP3炎性体信号通路相关分子在慢性阻塞性肺疾病急性加重(AECOPD)患者中的表达水平及其临床意义。[方法]选取河南大学淮河医院AECOPD患者48例作为实验组,对照组为我院体检中心的同期健康体检人员46例,用PCR法检测NLRP3炎性小体在实验组及对照组外周血单个核细胞(PBMC)的mRNA表达水平,比较NLRP3基因在两组中的表达差异性,用酶联免疫吸附试验(ELISA)检测下游炎症因子IL-1β和IL-18在两组中的表达水平。用肺功能仪测定两组研究对象的肺功能情况。[结果]AECOPD患者PBMC中的NLRP3 mRNA表达水平明显高于健康对照组水平,且二者差异具有统计学意义(P<0.05)。AECOPD患者血清IL-1β、IL-18和NLRP3水平明显高于健康对照组血清蛋白水平,且二者差异具有统计学意义(P<0.05)。AECOPD患者血清IL-1β、IL-18和NLRP3含量与肺功能指标FEV1/FVC、FEV1%呈负相关(P<0.05)。[结论]AECOPD患者NLRP3炎性小体水平升高,且与肺功能负相关,可能是评价患者病情轻重的重要指标。[Objective]Investigate the expression level of NLRP3 inflammasome signaling pathway related molecules in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and to discuss its clinical value.[Methods]In this study,48 patients with AECOPD in Huaihe hospital of Henan University were selected as the experimental group,and 46 cases having health check-up and no other respiratory disease were chosen as the control group in the same period.The mRNA levels of NLPR3 inflammasome of peripheral blood mononuclear cells in the two groups detected using quantitative real-time polymerase chain reaction(qRT-PCR).The concentration of NLRP3,IL-1βand IL-18 were measured by ELISA method in the experimental group and control group.Pulmonary function was measured by portable lung function meter.[Results]The expression of NLRP3 was significantly increased in peripheral blood mononuclear cells of at AECOPD patients mRNA levels(P<0.05).Furthermore,the serum IL-1β、IL-18 and NLRP3 levels in the AECOPD group were significantly higher than those in the control group(P<0.05).The serum IL-1β、IL-18 and NLRP3 levels in the AECOPD group were negatively correlated with the pulmonary function indexes(P<0.05).[Conclusion]The levels of NLRP3 inflammasome are related to the severity of disease in patients with AECOPD,which had certain clinical value in evaluating the severity of the disease.
