摘　　要：目的Chiari畸形合并寰枢椎脱位23例手术治疗探讨。方法选取2014年5月至2016年5月武警四川总队医院收治的23例Chiari畸形合并寰枢椎脱位患者进行研究,根据疾病情况采取治疗方式,8例患者采用后颅凹减压+硬膜扩大修补术,其余15例患者均采用后颅凹减压及植骨融合固定术,观察治疗效果。结果后颅凹减压+硬膜扩大修补术治疗的患者,6例略有好转,2例病情加重;后颅凹减压及植骨融合固定术治疗的患者,9例显著改善,无病情加重案例。结论Chiari畸形合并寰枢椎脱位治疗时,采取后颅凹减压及植骨融合固定术,能够取得较好效果。Objective To investigate the surgical treatment of 23 cases of Chiari malformation with atlantoaxial dislocation.Methods Totally 23 patients with Chiari malformation and Atlantoaxial dislocation were selected from May 2014 to May 2016,and 8 patients were treated with posterior Fossa decompression and dural dilatation,the other 15 cases were treated with posterior fossa decompression and bone graft fusion and fixation.Results 6 cases were slightly improved and 2 cases were worse after Posterior Fossa decompression and dural expansion repair,9 cases were significantly improved after posterior fossa decompression and bone graft fusion and fixation,and 9 cases were not worse.Conclusion The treatment of Chiari malformation combined with Atlantoaxial dislocation can obtain good results by decompression of posterior Fossa and bone grafting and fusion.
