彩色多普勒超声诊断椎动脉病变的临床研究

摘　　要：目的探讨彩色多普勒超声在椎动脉病变中的诊断价值。方法选取广东省茂名市农垦医院2018年10月至2019年9月收治的130例疑似椎动脉病变患者作为研究对象,根据检查方法不同分为彩超组及CT组,以经磁共振颈部血管成像(增强)检查为“金标准”进行价值分析。观察CT及彩色多普勒超声检查的声像图表现,并比较两种不同检查方式的病灶检出率、诊断符合率及颈动脉病变类型。结果以经磁共振颈部血管成像为“金标准”,其检出结果为130例,病灶检出率为100%。彩超组病灶检出率为100%,诊断符合率为99.23%,高于CT组的84.62%和76.92%;彩超组峰值流速降低,走行扭曲、变异,内径狭窄、闭塞、斑块及起源异常等颈动脉病变类型比例较CT组高(P<0.05)。结论彩色超声多普勒可提高椎动脉病变的病灶检出率和确诊率,且利于椎动脉类型的检测,值得推广应用。Objective To explore the value of color Doppler ultrasound in the diagnosis of vertebral artery disease.Methods Totally 130 patients with suspected vertebral artery disease from October 2018 to September 2019 in Nongken Hospital of Maoming City,Guangdong Province,were selected as the study objects.According to the different examination methods,they were divided into color Doppler ultrasound group and CT group.The value analysis was carried out with the“gold standard”of MR neck angiography(enhancement)examination.To observe the manifestations of CT and CDUS,and to compare the detection rate,diagnostic coincidence rate and the types of carotid artery lesions of the two different examination methods.Results Mri neck angiography was used as the"gold standard".The detection results were 130 cases and the detection rate was 100%.The detection rate of lesions in the color ultrasound group was 100%,and the diagnostic coincidence rate was 99.23%,higher than that of the CT group(84.62%,76.92%).In the color doppler ultrasound group,the peak flow velocity was decreased,and the proportion of carotid artery lesions,such as twist,variation,internal diameter stenosis,occlusion,plaque and abnormal origin,was higher than that in the CT group(P<0.05).Conclusion CDFI can improve the detection rate and diagnosis rate of vertebral artery lesions,and it is beneficial to the detection of vertebral artery types,which is worth popularizing.
