摘  要:目的探究和分析磁共振结合超声在胎儿中枢神经系统畸形中的诊断价值。方法选取2018年1月至2018年12月吉林市中心医院行产前胎儿中枢神经系统畸形筛查的20例孕妇作为研究对象,回顾性分析孕妇的临床资料,所有对象均接受了超声检查诊断及磁共振结合超声检查诊断,以最终终止妊娠实施引产检查结果作为金标准,对比两种方法对胎儿中枢神经系统畸形诊断的准确率。结果引产胎儿尸体解剖检查结果显示,脑室扩张5例、颅后窝池扩张5例、透明隔缺如4例、半椎体4例、蝴蝶椎2例;超声共检出脑室扩张3例、颅后窝池扩张4例、透明隔缺如2例、半椎体3例、蝴蝶椎2例;磁共振结合超声共检出脑室扩张4例、颅后窝池扩张5例、透明隔缺如3例、半椎体4例、蝴蝶椎2例,磁共振结合超声检查对胎儿中枢神经系统畸形诊断的准确率(90.00%)显著高于超声检查(70.00%),2组比较差异有统计学意义(P<0.05)。结论磁共振结合超声在胎儿中枢神经系统畸形中的诊断价值显著,不仅具有较高的诊断准确率,且还能鉴别区分出胎儿中枢神经系统畸形类型。Objective To explore and analyze the diagnostic value of magnetic resonance combined with ultrasound in fetal central nervous system malformations.Methods From January 2018 to December 2018,totally 20 pregnant women who underwent prenatal fetal central nervous system malformation screening in our hospital were selected as the research subjects.The clinical data of pregnant women were retrospectively analyzed.All subjects received ultrasound diagnosis and the diagnosis of magnetic resonance combined with ultrasound examination based on the results of the labor termination examination for the final termination of pregnancy as the gold standard,and the accuracy of the two methods for the diagnosis of fetal central nervous system malformation is compared.Results The autopsy results of the induced fetus showed 5 cases of ventricular dilation,5 cases of posterior cranial fossa dilation,4 cases of transparent septal defect,4 cases of hemivertebral body,and 2 cases of butterfly vertebra.3 cases of ventricular dilation were detected by ultrasound.There were 4 cases of posterior cranial fossa dilation,2 cases of transparent septa,3 cases of hemivertebral bodies,and 2 cases of butterfly vertebrae.Magnetic resonance combined with ultrasound detected 4 cases of ventricular dilation,5 cases of posterior cranial fossa pool dilation,and transparent septa.3 cases,4 cases of hemivertebrae,and 2 cases of butterfly vertebrae,the accuracy rate of diagnosis of fetal central nervous system malformation(90.00%)was significantly higher than that of ultrasound examination(70.00%)by the combination of magnetic resonance and ultrasound,and the comparison between the two groups was statistically significant(P<0.05).Conclusion Magnetic resonance combined with ultrasound is of great value in the diagnosis of fetal central nervous system malformation.It not only has a high diagnostic accuracy,but also distinguishes the fetal central nervous system malformation type.
