摘  要:目的 探讨儿童系统性红斑狼疮血脂特点及其危险因素.方法 回顾性分析2017年1月~2019年8月赣南医学院第一附属医院风湿免疫科收治的100例系统性红斑狼疮患儿的临床资料与血脂生化结果,总结其血脂特点与危险因素.结果 本组患儿的总胆固醇升高率为60.00%,高密度脂蛋白降低率为42.00%,甘油三酯升高率为67.00%,低密度脂蛋白升高率为46.00%;总胆固醇升高患儿的狼疮肾炎、自身免疫性血小板减少、大量蛋白尿、高敏C反应蛋白升高、激素治疗与血脂测定前1个月激素剂量超过1 g率均高于总胆固醇正常患儿,差异具有统计学意义(P<0.05);通过开展多因素分析可知,狼疮肾炎、大量蛋白尿与血脂测量前1个月激素总量超过1g属于儿童系统性红斑狼疮总胆固醇升高的独立危险因素(P<0.05);高敏C反应蛋白降低、自身免疫性血小板增加为儿童系统性红斑狼疮总胆固醇升高的保护因素(P<0.05).结论 系统性红斑狼疮患儿脂代谢紊乱高发,肾损害会明显影响总胆固醇水平.Objective To explore the characteristics and risk factors of blood lipid in children with systemic lupus erythematosus(SLE).Methods The clinical data and biochemical results of blood lipids of 100 SLE children admitted to the department of rheumatology and immunology,the First Affiliated Hospital of Gannan Medical College from January 2017 to August 2019 was analyzed retrospectively,and the characteristics and risk factors of blood lipids were summarized.Results The increase rate of total cholesterol was 60.00%,the decrease rate of high density lipoprotein was 42.00%,the increase rate of triglyceride was 67.00%,and the increase rate of low density lipoprotein was 46.00%.The rate of nephritis,autoimmune thrombocytopenia,massive albuminuria,high-sensitive C-reactive protein,hormone dosage>1 g in the first month before hormone therapy and blood lipid determination in children with increased total cholesterol were all higher than those in children with normal total cholesterol,and the differences were statistically significant(P<0.05).The multivariate analysis showed that lupus nephritis,massive albuminuria and hormone dosage>1 g in the first month before blood lipid determination were independent risk factors for the increase of total cholesterol in children with SLE(P<0.05);the decrease of high-sensitivity C-reactive protein and the increase of autoimmune platelets were protective factors for the increase of total cholesterol in children with SLE(P<0.05).Conclusion The incidence of lipid metabolism disorder in children with SLE is high,and renal damage will significantly affect the level of total cholesterol.
