摘　　要：目的 探讨锁扣带袢钛板在治疗胫骨髁间隆起骨折中的临床疗效.方法 选取本院2013年1月~2019年6月接收的胫骨髁间隆起骨折患者36例为研究对象,按照随机数表法将患膝随机分为两组:对照组接受关节镜下“8”字缝线固定手术治疗;研究组接受关节镜下锁扣带袢钛板固定手术治疗.术后9个月,根据X线、膝关节活动度、稳定性及Tegner评分、Lysholm评分、IKDC评分评估患者膝关节功能恢复情况来分析治疗效果.结果 36例患者术后9个月X线检查,对照组16例(88.89%)骨折愈合,2例(11.11%)骨折不愈合;研究组18例(100.00%)均骨性愈合,无骨折不愈合情况,术后膝关节活动度正常,无膝关节僵硬现象出现,前抽屉试验、Lachman试验、轴移试验均阴性,术后Tegner评分、Lysholm评分、IKDC评分均较对照组增高,具有显著性差异(P<0.05),表明治疗后研究组膝关节功能较对照组有明显改善.结论 胫骨髁间隆起骨折,采用锁扣带袢钛板固定胫骨髁间隆起骨折的临床治疗方法,其可以有效弥补“8”字缝线固定手术的不足,该方法具有骨折愈合率高,并发症发生率低、创伤小、恢复快等特点,能够让患者及早恢复膝关节功能,值得推广应用.Objective To explore the clinical effect of titanium plate with locking loop and belt carrier in tibial intercondylar eminence fractures.Methods 36 patients with tibial intercondylar eminence fractures who were admitted to our hospital from January 2013 to June 2019 were selected as study subjects.The affected knees were randomly divided into two groups according to the random number table method:The control group was given arthroscopic"8"-shaped suture fixation surgery,the study group was given arthroscopic titanium plate fixation surgery with locking loop and belt carrier.At 9 months after surgery,knee function recovery was evaluated based on X-rays,knee mobility,stability and Tegner score,Lysholm score,and IKDC score to analyze the effect of treatment.Results X-ray examination was performed in 36 patients 9 months after surgery.In the control group,there were 16 patients(88.89%)with fracture healing and 2 patients(11.11%)with non-healing fractures.All 18 cases(100.00%)in the study group showed fracture healing,without non-healing fractures.Postoperative knee joint activity was normal,and no knee stiffness was observed.The front drawer test,Lachman test,and axis shift test were all negative.Postoperative Tegner score,Lysholm score,and IKDC score were all higher than those in the control group,and the differences were statistically significant(P<0.05),showing that the knee function in the study group was significantly improved compared with the control group after treatment.Conclusion The clinical treatment method of tibial intercondylar eminence fractures by titanium plate fixation with locking loop and belt carrier can effectively compensate the deficiency of"8"-shaped suture fixation.The method has the characteristics of high fracture healing rate,low complication rate,small trauma,and fast recovery,and can enable patients to recover knee function early,which is worthy of promotion and application.
