肩关节镜手术治疗肩袖损伤的临床疗效分析

摘　　要：目的 探讨肩关节镜手术治疗肩袖损伤的临床疗效.方法 选取2018年1月~2019年11月在我院骨科治疗的60例肩袖损伤患者为研究对象,按不同术式分为两组,对照组行肩关节镜下肩袖清理术,观察组使用肩关节镜肩袖修复术,采用美国加州大学肩关节评分(UCLA)和Constant-Murley肩关节功能评分评估肩关节功能恢复情况,采用美国肩与肘协会评分(ASES)标准评估疗效.结果 观察组术后7d、术后3个月的前侧屈曲活动、外展屈曲活动、体侧外旋活动、疼痛、功能以及UCLA总分均明显高于对照组(P<0.05);观察组术后7d、术后3个月的疼痛、活动度、功能活动、肌力以及Constant-Murley总分均高于对照组(P<0.05);观察组治疗优良率为100.00%,明显高于对照组的86.67% (P<0.05).结论 肩关节镜手术治疗肩袖损伤的临床疗效显著,能有效减轻疼痛,促进肩关节活动度的恢复,提高肩关节功能,且创伤小,术后并发症发生率低,能加快患者的恢复速度,具有积极的临床意义.Objective To investigate the clinical efficacy of shoulder arthroscopy in the treatment of rotator cuff injury.Methods 60 patients with rotator cuff injury who were treated in the department of orthopedics in our hospital from January 2018 to November 2019 were selected as the research subjects.They were divided into two groups according to different operation methods.The control group was given rotator cuff cleanup under shoulder arthroscopy,and the observation group was given rotator cuff repair under shoulder arthroscopy.The university of Califomia at Los Angeles shoulder rating scale(UCLA)and constant-murley shoulder function score were used to assess the recovery of shoulder function.The efficacy was evaluated using the American shoulder and elbow association score(ASES).Results The anterior flexion activity,abduction flexion activity,body lateral rotation activity,pain,function,and total UCLA scores in the observation group were significantly higher than those in the control group at 7 days and 3 months after surgery(P<0.05).The pain,mobility,functional activity,muscle strength and overall Constant-Murley scores in the observation group at 7 days and 3 months after surgery were higher than those in the control group(P<0.05).The excellent and good treatment rate in the observation group was 100.00%,which was significantly higher than that of 86.67%in the control group(P<0.05).Conclusion The clinical effect of shoulder arthroscopic surgery in the treatment of rotator cuff injury is significant.It can effectively reduce pain,promote the recovery of shoulder joint mobility,improve shoulder function,and have less trauma,and the incidence of postoperative complications is low.This surgery can accelerate the recovery rate of patients,and has a positive clinical significance.
