无创通气治疗急性左心衰竭的疗效研究

摘　　要：目的探讨无创通气治疗急性左心衰竭的疗效。方法选取2014年1月—2018年1月我院急性左心衰竭患者共60例,分为治疗组和对照组,对照组常规给予解除诱因、面罩高流量吸氧、强心、利尿、扩血管治疗;治疗组在常规治疗的基础上加用无创面罩正压通气治疗,对比2组治疗后1 h、24 h的呼吸频率(RR)、心率(HR)、平均动脉压(MAP)、指氧饱和度(SpO2)及临床疗效。结果2组1 h后SpO2及治疗1 h、24 h后的MAP比较无明显差异(P>0.05);治疗组治疗1 h及24 h后的心率、呼吸频率明显低于对照组,而治疗24 h后SpO2明显高于对照组(P<0.05)。治疗组总有效率为93.3%,明显高于对照组的66.7%,差异有统计学意义(P<0.05)。结论在常规治疗的基础上联合无创通气,能快速降低急性左心衰竭患者的心率及呼吸频率,降低氧耗,并且能显著提高临床疗效。Objective To investigate the effect of acute left heart failure in elderly patients with non-invasive ventilation.Methods From January 2014 to January 2018,60 patients with acute left heart failure were selected Cases were divided into the treatment group and the control group.In the control group,the inducement was relieved,the mask was used for high flow oxygen,heart strengthening,diuresis and vasodilation;in the treatment group,the positive pressure ventilation without wound mask was added on the basis of conventional treatment,and the two groups were observed for 1 hour and 24 hours after treatment At the same time,the respiratory rate(RR),heart rate(HR),mean arterial pressure(map),finger oxygen saturation(SpO2)and other indicators and clinical manifestations were compared to determine the clinical efficacy.Results There was no significant difference between the treatment group and the Control Group(P>0.05),but there was no significant difference between the treatment group and the Control Group(P>0.05)The heart rate and respiratory rate of the treatment group were significantly lower than those of the control group(P<0.05),and the total effective rate of the treatment group(93.3%)was significantly higher than that of the Control Group(66.7%),the difference was statistically significant(P<0.05).Conclusion Combined with noninvasive ventilation on the basis of conventional treatment,it can rapidly reduce the heart rate and respiratory frequency of patients with acute left heart failure,reduce the oxygen consumption of patients,and significantly improve the clinical efficacy of patients with acute left heart failure.
