摘 要:目的 观察不稳定型心绞痛(UAP)患者血清微小RNA(miRNA)-146a rs2910164基因多态性的分布特征,探讨miRNA-146a表达水平与冠状动脉(冠脉)病变严重程度的相关性.方法 入选2018年6月至12月于徐州医科大学附属医院心内科就诊的UAP患者100例(UAP组),根据Gensini评分评估患者冠脉病变严重程度,按四分位法将入选的UAP患者分为4个亚组,同期纳入冠脉造影无明显异常的100例患者作为正常对照组,检测miRNA-146a rs2910164基因的多态性及表达水平,观察两组人群中miRNA-146a基因多态性的分布情况,并评估UAP各亚组患者miRNA-146a表达水平与冠脉Gensini评分的关系.结果 与正常对照组相比,CC+CG基因型在UAP患者中更常见,且CC+CG基因型携带者比非携带者罹患不稳定型心绞痛的风险高2.389倍(χ2=5.093,P=0.024,OR=2.389,95%CI:1.170-4.878).UAP组miRNA-146a表达水平明显高于正常对照组(Z=8.664,P<0.001),其各亚组中也存在差异(t/χ2=4.613,P=0.023);随着miRNA-146a表达水平的升高,Gensini评分也随之升高.结论 血液中miRNA-146a基因型在人群中分布存在差异,与GG基因型相比,携带CC+GC基因型的人群罹患UAP的可能性更大.UAP患者血液中miRNA-146a表达水平明显高于正常人,且随着miRNA-146a表达水平的升高,Gensini评分也呈增高趋势.Objective To observe the distribution characteristics of rs2910164 gene polymorphism of serum micro-RNA(miRNA)-146a,and discuss the correlation between expression of miRNA-146a and severity of coronary artery lesions in patients with unstable angina pectoris(UAP).Methods UAP patients(n=100,UAP group)were chosen from Department of Cardiology in Affiliated Hospital of Xuzhou Medical University from June 2018 to Dec.2018.The severity of coronary artery lesions was reviewed according to Gensini scoring and UAP patients were divided into 4 subgroups according to quartile method.At the same time the patients(n=100)without significant outcomes of abnormal coronary angiography were chosen into normal control group.The miRNA-146a rs2910164 gene polymorphism and expression were detected.The distribution of miRNA-146a gene polymorphism was observed in 2 groups.The relationship between miRNA-146a expression and coronary artery Gensini scores was reviewed in all subgroups.Results Compared with normal control group,CC+CG genotype was more common in UAP group,and UAP risk was 2.389 times higher in CC+CG genotype carriers than that in non-carriers(χ2=5.093,P=0.024,OR=2.389,95%CI:1.170~4.878).The expression of miRNA-146a was significantly higher in UAP group than that in normal control group(Z=8.664,P<0.001),and there was difference among different subgroups(t/χ2=4.613,P=0.023).Along with the expression of miRNA-146a increased,Gensini scores also increased.Conclusion The distribution of miRNA-146a genotypes is different in different population.The risk of UAP is higher in CC+GC genotype carriers than that in GG genotype carriers.The expression of miRNA-146a is significantly higher in UAP patients than that in normal people,and along with the increases of miRNA-146a expression,Gensini scores will increase.
