穴位贴敷联合百合固金汤加减治疗慢性支气管炎肺肾阴虚证的疗效和机制研究

摘　　要：目的探究穴位贴敷联合百合固金汤加减治疗慢性支气管炎肺肾阴虚证的疗效和相关机制。方法选取2017年4月-2019年4月本院诊治的慢性支气管炎肺肾阴虚型患者104例,按随机数字表法分为2组,均52例。对照组给予百合固金汤加减治疗,研究组给予穴位贴敷联合百合固金汤加减治疗,均治疗4个疗程。比较2组治疗前后的中医证候积分、炎症因子水平、中医证候疗效及安全性。结果治疗后,研究组主症、次症积分及血清TNF-α、IL-8水平低于对照组及治疗前,IFN-γ、IL-10水平高于对照组及治疗前,差异有统计学意义(P<0.05)。研究组治疗总有效率(94.23%)高于对照组(78.85%),差异有统计学意义(P<0.05)。治疗过程2组患者均无明显不良反应出现。结论穴位贴敷联合百合固金汤加减治疗慢性支气管炎肺肾阴虚证安全性高,可显著缓解中医症状,提高疗效;其机制可能与上调IFN-γ、IL-10表达及下调TNF-α、IL-8水平有关。Objective To explore the effect and related mechanism of acupoint application combined with Baihe Gujin decoction in the treatment of patients with chronic bronchitis with lung kidney yin deficiency syndrome.Methods 104 patients with chronic bronchitis with lung and kidney yin deficiency syndrome diagnosed in our hospital from April 2017 to April 2019 were enrolled in this study.All patients were divided into two groups according to the random number table.The control group was treated with Baihe Gujin decoction,and the study group was given acupoint sticking combined with Baihe Gujin decoction for treatment.Comparison of TCM syndrome scores and inflammatory factors levels before and after treatment,TCM syndrome efficacy and safety.Results After treatment,the main symptoms,secondary syndrome scores and serum TNF-α,IL-8 levels in the study group were lower than those in the control group and before treatment.The levels of IFN-γand IL-10 were higher than those in the control group and before treatment.The difference was statistically significant(P<0.05).The total effective rate of treatment in the study group(94.23%)was higher than that in the control group(78.85%),and the difference was statistically significant(P<0.05).There were no significant adverse reactions in the two groups of patients during the treatment.Conclusion Acupoint sticking combined with Baihe Gujin decoction for the treatment of chronic bronchitis with lung kidney yin deficiency syndrome patients with high safety can significantly alleviate the symptoms of TCM and improve the curative effect.The mechanism may be related to up-regulation of IFN-γ,IL-10 expression and down-regulation of TNF-α,IL-8 levels.
