摘　　要：目的 椎间盘纤维环后方高信号区(HIZ)是在磁共振成像(MRI)上经常观察到的一种脊柱疾病的影像学指征.HIZ首次提出时被定义为T2加权成像(T2WI)上的高信号表现,至今已有近30年的研究历史,丰富临床研究证据的同时衍生出多种HIZ征象的相关研究(颈椎HIZ、纤维环前方HIZ、双HIZ等).此外,针对HIZ的物质基础(如炎性肉芽组织和钙化组织)及其所代表临床意义的研究也逐渐增多.且正以更为多元化的角度分析HIZ所代表的意义,揭露HIZ现象的本质,进而用于指导临床的诊断、治疗及康复训练等.但研究的证据等级不高,相当一部分可能存在较大偏倚,后期仍需行更多高证据等级的诊断研究试验.Objective The high-intensity zone(HIZ)inferior to annulus fibrosus disci intervertebralis is a kind of imaging indications of spinal disease often observed by magnetic resonance imaging(MRI).When HIZ was proposed at the fist time,it was defined as high signal performance on T2 weighted image(T2WI).So far,it has a research history of nearly 30 years,which enriched the clinical research evidence and derived related research on various HIZ signs(cervical HIZ,anterior HIZ,double HIZ,etc.).In addition,studies on material basis of HIZ(such as inflammatory granulation tissue and calcified tissue)and on clinical significance of HIZ have gradually increased.The current researches analyze the significance of HIZ from diversified perspectives to investigate the nature of HIZ phenomenon,and then to guide clinical diagnosis,treatment and rehabilitation training.However,evidences in these studies could not reach the research criterion comprehensively,which have a considerable number of large biases.Therefore,more high-level diagnostic research trials are still needed in the further study.
