成人后孟氏骨折脱位合并桡骨头骨折的手术策略及临床预后探讨

摘　　要：目的 探讨成人后孟氏骨折脱位合并桡骨头骨折的手术策略及临床预后.方法 50例成人后孟氏骨折脱位合并桡骨头骨折患者,随机分为对照组和观察组,每组25例.对照组采用切开复位尺骨骨折与闭合复位桡骨骨折的治疗方法,观察组采用切开复位尺骨骨折与桡骨骨折复位及韧带修复的治疗方法.比较两组患者近期和远期预后效果以及不良事件发生情况.结果 术后1年,观察组与对照组的临床优良率分别为92.00%与88.00%,组间比较差异无统计学意义(P>0.05).术后2年,观察组的临床优良率为92.00%,高于对照组的68.00%,差异有统计学意义(P<0.05).观察组与对照组的不良事件发生率分别为4.00%与8.00%,组间比较差异无统计学意义(P>0.05).结论 成人后孟氏骨折脱位合并桡骨头骨折患者采用切开复位尺骨骨折与桡骨骨折复位及韧带修复的治疗方法,远期效果显著,推荐在临床中选用.Objective To discuss the surgical strategy and clinical prognosis of posterior Monteggia fracture-dislocation in adults with radial head fracture.Methods A total of 50 adult patients with posterior Monteggia fracture-dislocation and radial head fracture were randomly divided into control group and observation group,with 25 cases in each group.The control group was treated by open reduction of ulna fracture and closed reduction of radius fracture,and the observation group was treated by open reduction of ulna fracture and radial fracture and ligament repair.The short-term and long-term prognostic effects and occurrence of adverse events were compared between the two groups.Results 1 year after surgery,the clinical excellent-good rate of the observation group and control group were 92.00%and 88.00%,and the difference was statistically significant(P>0.05).2 years after surgery,the clinical excellent-good rate of the observation group was 92.00%,which was higher than that of the control group 68.00%,and the difference was statistically significant(P<0.05).The incidence of adverse events of the observation group and control group were 4.00%and 8.00%,and the difference was not statistically significant(P>0.05).Conclusion Open reduction of ulna fracture and radial fracture and ligament repair shows remarkable long-term effect for adult patients with posterior Monteggia fracture-dislocation and radial head fracture,and it is recommended to be selected in clinical practice.
