摘  要:目的 研究血清特异性免疫球蛋白E(IgE)联合免疫球蛋白G(IgG)检测在早期判断儿童过敏性疾病中的应用价值.方法 90例过敏性疾病患儿作为研究对象,应用酶联免疫方法对血清中食物过敏原特异性IgG进行测定,并应用免疫印迹法对血清中吸入性过敏原与食物过敏原特异性IgE进行测定.分析食物过敏原特异性IgG和IgE检测结果及其一致性,并分析吸入性过敏原特异性IgE检测结果.结果 90例过敏性患儿经食物过敏原特异性IgG检测呈阳性84例,阳性检出率为93.33%(84/90),主要阳性食物为牛奶、鸡蛋与大豆,重度敏感食物过敏原为牛奶与鸡蛋.食物过敏原特异性IgE检测呈阳性患儿40例(44.44%),主要阳性食物为牛羊肉、牛奶,重度敏感食物过敏原为鸡蛋、鱼虾蟹.经Kappa检验显示,特异性IgE与特异性IgG检测结果不一致(U=0.030,Kappa=0.012),而食物过敏原特异性IgG阳性率高于特异性IgE阳性率,差异具有统计学意义(χ2=50.184,P<0.05).吸入性过敏原特异性IgE抗体阳性明确31例,阳性检出率为34.44%(31/90),主要阳性过敏原为尘螨、点青霉分支孢与花粉,重度敏感吸入性过敏原为尘螨.结论 儿童过敏性疾病多是由食物过敏原与吸入性过敏原引起,而早期给予联合检测食物过敏原特异性IgE、IgG效果显著,能够明确食物过敏原,及时制定环境控制和药物干预方案,对防治儿童过敏性疾病具有重大意义.Objective To study the practical value of serum-specific immunoglobulin E(IgE)combined with immunoglobulin G(IgG)detection in early detection of allergic diseases in children.Methods There were 90 children with allergic diseases taken as the research subjects.The enzyme-linked immunoassay method was used to determine the food allergen-specific IgG in the serum,and the immunoblot method was used to determine the serum allergen inhalation and food allergen-specific IgE.The results of food allergen specific IgG and IgE and their consistency were analyzed,and the inhaled allergen specific IgE test results was analyzed.Results Of the 90 children,84 cases were positive for the food allergen-specific IgG test,and the positive detection rate was 93.33%(84/90).The main positive foods were milk,eggs and soybeans,and the most sensitive food allergens were milk and eggs.40 children(44.44%)were positive for food allergen-specific IgE.The main positive foods were beef,mutton and milk,and the most sensitive food allergens were eggs,fish,shrimp and crab.Kappa test showed that the specific IgE and specific IgG test results were inconsistent(U=0.030,Kappa=0.012),and the food allergenspecific IgG positive rate was higher than the specific IgE positive rate,the difference was statistically significant(χ2=50.184,P<0.05).Inhaled allergen-specific IgE antibodies were positive in 31 cases,with a positive detection rate of 34.44%(31/90).The main positive allergens were dust mite,Paecilomyces punctatus and pollen,and the severe sensitive inhaled allergens were dust mite.Conclusion Most of allergic diseases in children are caused by food allergens and inhaled allergens,and the early combined detection of food allergen specific IgE and IgG has a significant effect,which can identify food allergens,timely develop environmental control and drug intervention programs.It is of great significance for the prevention and control of allergic diseases in children.
