摘 要:目的了解辽宁省县级公立医院卫生资源配置情况及其公平性,为优化县级公立医院资源配置提供建议,为分级诊疗的实施奠定资源基础。方法应用集中指数分解法、间接标准化集中指数法等进行资源的公平性分析。结果2016年辽宁省平均每个地区县级公立医院执业(助理)医师5.89人/万人,注册护士7.12人/万人,医护比为1∶1.25,实际床位数18.21张/万人,病床使用率偏低,万元医疗设备9台/万人;整体公平性较好,需求因素对不公平的贡献率大于经济因素。结论适当提升各地县级公立医院主要资源配置水平,促进资源的合理流动,注重需求因素对公平性的影响。Objective To understand the situation and fairness of health resource allocation in county-level public hospitals in a province,to provide suggestions for optimizing the resource allocation of county-level public hospitals,and to lay a resource foundation for the implementation of hierarchical diagnosis and treatment. Methods The fairness analysis of resources was carried out by using concentration index decomposition method and indirect standardized concentration index method. Results In 2016, the average number of licensed(assistant) doctors and registered nurses in county-level public hospitals in each region of this province was 5.89/10 000 and 7.12/10 000, with a ratio of 1∶1.25. Overall fairness is better, and the contribution rate of demand factors to unfairness is greater than that of economic factors. Conclusion The main resource allocation level of county-level public hospitals should be appropriately improved, the rational flow of resources should be promoted, and the influence of demand factors on equity should be paid attention to.
