摘　　要：慢性病严重影响了患者的生活质量且占用了大量的优质医疗资源,中医中药和中医诊疗技术在慢性病治疗方面有独特的见解和优势,中医医联体家庭医生团队慢性病管理模式可以解决慢性病难根治、易复发的问题,对改善慢性病患者的生活质量有重要意义。Chronic diseases seriously affect the quality of life of patients and occupy a large number of high-quality medical resources. Traditional Chinese Medicine(TCM) and TCM diagnosis and treatment technology have unique views and advantages in the treatment of chronic diseases. The chronic disease management mode of family doctor team in TCM alliance can solve the problem of chronic disease,which is difficult to cure and easy to recur. It is of great significance to improve the quality of life of patients with chronic diseases.
