老年患者髋关节置换术后髋关节疼痛的影响因素分析

摘　　要：目的分析老年患者髋关节置换术后发生髋关节疼痛的影响因素。方法选取2017年3月至2018年10月于河南科技大学第二附属医院接受髋关节置换术的老年患者106例为研究对象,统计其术后半年疼痛发生率,比较疼痛组和未疼痛组性别、年龄、体质指数、病理类型、手术入路方式、术后白细胞计数(WBC)水平、术后C反应蛋白(CRP)水平、假体类型及术后髋关节活动时间等,采用Logistic回归分析法分析影响髋关节置换术后髋关节疼痛的因素。结果髋关节置换术后髋关节疼痛的发生率为36.79%(39/106)。疼痛组和非疼痛组年龄、假体类型、术后髋关节活动时间、术后CRP水平、术后WBC水平比较差异有统计学意义(P<0.05),两组性别、体质指数、病理分型、手术入路方式比较差异未见统计学意义(P>0.05)。年龄>65岁、使用非骨水泥型假肢、术后髋关节活动时间>2 d、术后WBC>10×10^9/L、术后CRP>10 mg/L是髋关节置换术后髋关节疼痛的影响因素。结论老年人髋关节置换术后髋关节疼痛受诸多因素影响,其中年龄>65岁、使用非骨水泥型假肢、术后髋关节活动时间>2 d、术后WBC>10×10^9/L、术后CRP>10 mg/L是主要影响因素。Objective To investigate the influencing factors of hip pain after hip replacement in the elderly patients.Methods A total of 106 elderly patients who underwent hip replacement in the Second Affiliated Hospital of Henan University of Science and Technology from March 2017 to October 2018 were selected.The incidence of pain within half a year after operation was recorded.The data of gender,age,body mass index,pathological type,operation mode,white blood cell(WBC),C-reactive protein(CRP),prosthesis type and activity time of hip joint were compared between pain group and non pain group.Logistic regression analysis were used to analyze the influencing factors of hip pain after hip replacement.Results The incidence of hip pain after hip replacement was 36.79%(39/106).Age,prosthesis type,postoperative hip joint activity time,postoperative CRP level and postoperative WBC level had statistic difference between pain group and non pain group(P<0.05).While no statistic difference was found in the gender,body mass index,pathological classification or surgical approach between pain group and non pain group(P>0.05).Age over 65 years,uncemented prosthesis,postoperative hip joint activity time>2 d,postoperative WBC>10×10^9/L,and postoperative CRP>10 mg/L were the influencing factors of hip pain after hip replacement.Conclusions Hip pain after hip replacement in the elderly can be affected by many factors,and the main influencing factors are age>65 years,uncemented prosthesis,postoperative hip joint activity time>2 d,postoperative WBC>10×10^9/L,and postoperative CRP>10 mg/L.
