硒酵母联合左甲状腺素钠治疗桥本甲状腺炎伴甲状腺功能减退的效果观察

摘　　要：目的探讨硒酵母联合左甲状腺素钠治疗桥本甲状腺炎伴甲状腺功能减退患者的效果。方法回顾性分析德州市人民医院内分泌门诊2018年2月至2019年3月收治的85例桥本甲状腺炎伴甲状腺功能减退患者的临床资料,根据药物治疗方案分为对照组(左甲状腺素钠,42例)与观察组(硒酵母+左甲状腺素钠,43例)。两组均连续治疗90 d,观察两组临床治疗效果,对比治疗前后甲状腺功能指标及甲状腺过氧化酶抗体、甲状腺球蛋白抗体变化,记录两组不良反应发生情况。结果观察组治疗总有效率(93.02%,40/43)高于对照组(76.19%,32/42),P<0.05;治疗后,观察组甲状腺素水平及三碘甲状腺元氨基酸水平[(17.79±4.84)、(5.74±1.18)pg/ml]高于对照组[(14.36±4.28)、(4.12±0.96)pg/ml],促甲状腺素水平[(6.32±2.43)mIU/L]低于对照组[(9.26±2.54)mIU/L],P<0.05;观察组甲状腺过氧化酶抗体及甲状腺球蛋白抗体[(329.45±62.58)、(216.78±36.83)IU/ml]低于对照组[(534.87±73.54)、(356.97±48.79)IU/ml],P<0.05;观察组不良反应发生率为11.63%(5/43),对照组为9.52%(4/42),组间比较差异未见统计学意义(P>0.05)。结论桥本甲状腺炎伴甲状腺功能减退患者采用硒酵母联合左甲状腺素钠治疗效果显著,可有效提高甲状腺功能,减少甲状腺抗体,且可有效控制不良反应发生率。Objective To investigate the therapeutic effects of selenium yeast combined with levothyroxine sodium on Hashimoto thyroiditis patients with hypothyroidism.Methods The clinical data from 85 patients with Hashimoto thyroiditis and hypothyroidism admitted to Dezhou People’s Hospital from February 2018 to March 2019 were retrospectively analyzed.According to the drug treatment plans,patients were divided into the control group(42 cases treated with levothyroxine sodium)and observation group(43 cases treated with selenium yeast and levothyroxine sodium).Both groups were treated continuously for 90 d,and the clinical effects of the two groups were observed.The changes of thyroid function index,thyroid peroxidase antibody and thyroglobulin antibody of the two groups before and after treatment were compared.And the adverse reactions of the two groups were recorded.Results The total effective rate of the observation group(93.02%,40/43)was higher than that of the control group(76.19%,32/42),P<0.05.After treatment,the levels of thyroxine and triiodothyronine amino acid of the observation group were(17.79±4.84)pg/ml and(5.74±1.18)pg/ml,respectively,which were higher than(14.36±4.28)pg/ml and(4.12±0.96)pg/ml of the control group(P<0.05).And the levels of thyrotropin in the observation group was(6.32±2.43)mIU/L,lower than(9.26±2.54)mIU/L in the control group(P<0.05).The levels of thyroid peroxidase antibody and thyroglobulin antibody in the observation group were(329.45±62.58)IU/ml and(216.78±36.83)IU/ml,which were significantly lower than(534.87±73.54)IU/ml and(356.97±48.79)IU/ml in the control group(P<0.05).The incidence of adverse reactions was 11.63%(5/43)in the observation group,and 9.52%(4/42)in the control group,but there was no statistically significant difference between the two groups(P>0.05).Conclusions Selenium yeast combined with levothyroxine sodium can effectively improve thyroid function,reduce thyroid antibodies and effectively control the incidence of adverse reactions in Hashimoto thyroiditis pa
