社区糖尿病分阶段达标管理模式对血糖控制不佳的2型糖尿病患者的疗效分析

摘　　要：目的探讨社区糖尿病分阶段达标管理模式(SDTM)对血糖控制不佳的2型糖尿病(T2DM)患者的疗效。方法将240例血糖控制不佳的T2DM患者随机分为两组。对照组(n=120)按照规范进行管理,研究组(n=120)采用社区糖尿病"5+1"SDTM追踪管理模式。比较两组血糖控制、并发症检查率及生活质量改善情况。结果管理12个月后,两组FPG、2hPG及HbA1c下降,血压、LDL-C达标率和糖尿病并发症筛查率升高,研究组改善程度明显优于对照组(P<0.05);管理12个月后,研究组WHOQOL-BREF量表的生理、心理和社会关系维度评分高于对照组(P<0.05);研究组签约医生就诊率、预约门诊率和签约服务满意率较对照组升高(P<0.05)。结论SDTM模式下社区T2DM管理有助于提高血糖控制不佳患者的血糖水平,增强自我管理依从性,改善生存质量。OBJECTIVE To explore the clinical effect of grading management mode of diabetes in communities in patients with type 2 diabetes mellitus with poor blood glucose control.METHODS 240 T2DM patients with poor glycemic control were randomly divided into two groups.The control group(n=120)was managed according to the standard,while the research group(n=120)adopted the"5+1"SDTM tracking management model of community diabetes.Blood glucose control,complication rate and quality of life improvement were compared between these two groups.RESULTS After 1 year of management,FPG,2 hPG and HbA1 c in these two groups were decreased,blood pressure,LDL-C compliance rate and diabetes complication screening rate were increased,and the improvement degree of the research group was significantly better than the control group(P<0.05).After 1 year of management,the physiological,psychological and social relationship scores of WHOQOL-BREF scale in the research group were higher than the control group(P<0.05).The treatment rate,outpatient appointment rate and service satisfaction rate of contracted doctors in the study group were higher than the control group(P<0.05).CONCLUSION Community T2DM management in STDM mode was helpful to improve blood glucose control in T2 DM patients with poor blood glucose control,enhance self-management compliance,and improve quality of life.
