摘  要:目的探讨超声心动图对系统性红斑狼疮(SLE)及感染性心内膜炎(IE)赘生物的鉴别诊断价值。方法抽取2010年7月至2017年7月河南科技大学第一附属医院收治的临床诊断为IE者46例,另以同期收治的经临床确诊的SLE患者32例作为对照。比较其超声心动图检查结果的差异。结果32例SLE患者中,23例患者超声心动图检出赘生物,检出率为71.9%;9例患者超声心动图呈瓣叶增厚。46例IE患者中,39例检出心脏赘生物,检出率为84.7%,7例心脏超声图像提示瓣叶增厚,两组心脏赘物检出率比较差异未见统计学意义(P>0.05);超声对赘生物长径及瓣周并发症比较差异有统计学意义(P<0.05)。结论超声心动图可以早期快速发现SLE诱发的瓣膜受累时特征性的疣状赘生物,为IE及SLE的治疗及疗效观察提供有价值的参考。Objective To analyze the value of echocardiography in the differential diagnosis of systemic lupus erythematosus (SLE) and infective endocarditis (IE).Methods A total of 46 patients with infective endocarditis were selected form July 2010 to July 2017 in the First Affiliated Hospital of Henan University of Science and Technolog, and 32 patients with SLE who were diagnosed clinically in the same period were selected as the control group. Comparison of echocardiographic results between the two groups was performed.Results Among the 32 patients with SLE, vegetations were detected by echocardiography in 23 patients, and the detection rate was 71.9%. Echocardiography of 9 patients showed a thickening of leaf blade. Among the 46 patients with IE, 39 cases had vegetations detected by ultrasonography, and the detection rate was 84.7%. Echocardiography of 7 patients showed a thickening of the lobe. There was no significant difference in detection rate of vegetations between the two groups (P>0.05);there was a significant difference in the major diameter of the vegetations and the complications around the valve between the two groups (P<0.05).Conclusions Echocardiography can quickly detect the featured verrucous vegetations in valve involved by SLE. Echocardiography can provide valuable reference for the treatment and assessment on therapeutic effect of IE and SLE.
