摘　　要：目的探讨常规器械改良经脐单孔腹腔镜胆囊切除术的临床效果、可行性和可推广性。方法抽取2014年3月至2019年2月于四川省宜宾市第一人民医院采用常规器械行改良经脐单孔腹腔镜胆囊切除术者816例为观察组,另选取同期行传统三孔法腹腔镜胆囊切除术者820例为对照组,比较两组手术指标、术后效果及住院费用。结果两组患者术后恢复顺利,未见出血、胆漏、腹腔感染及切口疝等并发症。两组术中出血量、术后住院时间、住院费用比较,差异未见统计学意义(P>0.05)。观察组手术时间为(40.3±11.2)min、VAS评分为(2.3±1.4)分、腹壁满意度为(4.8±0.5)分,与对照组[(31.2±8.4)min,(5.0±1.5)分、(3.9±0.4)分]比较差异有统计学意义(P<0.05)。结论常规器械改良经脐单孔腹腔镜胆囊切除术创伤小,术后脐部切口瘢痕隐蔽,患者疼痛感觉轻且对治疗及美容效果满意。ObjectiveTo investigate the effect of clinical application,feasibility and generalization of improved transumbilical single-pore laparoscopic cholecystectomy(TUSILC)with conventional instruments.MethodsA total of 816 patients underwent improved TUSILC with conventional instruments(observation group)and 820 patients received conventional laparoscopic treatment(control group)from March 2014 to February 2019 in the First People’s Hospital of Yibin were selected.The surgical indicators,postoperative results,and hospitalization expenses were compared between the two groups.ResultsAll patients recovered well and no complications such as hemorrhage,bile leakage,abdominal infection and incisional hernia occurred.There were no significant differences in blood loss,postoperative hospitalization time and hospitalization expenses between the two groups(P>0.05).The operation time in the observation group was(40.3±11.2)minutes,the postoperative pain score was(2.3±1.4)points,and the recovery satisfaction degree of navel incision was(4.8±0.5)points,which was statistically significant compared with the(31.2±8.4)min,(5.0±1.5)points and(3.9±0.4)points in the control group(P<0.05).ConclusionsImproved transumbilical single-pore laparoscopic cholecystectomy by conventional instruments has small surgical trauma and invisible scars after surgery.The patients feel less pain and are satisfied with the treatment cosmetic result.
